DESCRIPTION OF SERVICES The Contractor shall furnish all labor, material, supplies, equipment, and qualified personnel to provide on-site diagnostic medical physics support or services, including annual (March and July) on-site radiation compliance testing and performance monitoring services of x-ray producing equipment for the William S. Middleton Memorial VA Hospital, 2500 Overlook Terrace, Madison WI and the Rockford Community Based Outpatient Clinic (CBOC) 816 Featherstone Road, Rockford, IL. The Contractor shall adhere to the VHA Handbook 1105.04, Fluoroscopy Safety, dated July 6, 2012.
