- Additional questions and answers for RFQ 79331FB This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Centers for Disease Control and Prevention (CDC), National Institute for Occupational Safety and Health (NIOSH), National Personal Protective Technology Laboratory (NPPTL), located near Pittsburgh, Pennsylvania, has a need for an environmental chamber. It will be constructed at the NIOSH facility near Pittsburgh in accordance with the specifications attached to this solicitation. The solicitation number for this requirement is 79331FB and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. QUOTERS ARE ADVISED AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their proposals. Therefore, the initial quote should contain the quoter's best terms. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. THIS IS A TOTAL SMALL BUSINESS SET-ASIDE. The North American Classification System (NAICS) code for this procurement is 332311 with a small business size standard of 500 employees. Quotes are due on Tuesday February 10, 2015 by 1:00 PM EST and must be sent to the attention of Florence Black via email at fpblack@cdc.gov. Evaluation and award of the purchase will be performed in accordance with applicable sections of FAR Part 12 Commercial Items and FAR Part FAR 13 Simplified Acquisition Procedures. The Offeror must have an active registration with the System for Account Management (SAM). The government intends to issue a firm, fixed price purchase order in accordance with FAR Part 13. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-79. Note to offerors: Funds are not presently available for this requirement. The Government's obligation under this RFQ is contingent upon the availability of appropriated funds, and no award will be made until those funds have been identified. No legal liability on the part of the Government for any payment may arise until funds are made available and a purchase order, signed by an authorized Contracting Officer, is issued. Special Notice: Any vendor wishing to see the area where the installation will occur is strongly encouraged to attend a site visit scheduled for Thursday, January 15, 2015 at 9:00 AM. Because access to CDC facilities is restricted, you must email your intent to attend the site visit no later than 1:00 PM, Tuesday, 01/13/15, and the email must include the names and contact phone numbers for all persons who will attend. The period of performance for this service contract will start 12 months after final installation and testing of the environmental test chamber. Although funding for the maintenance agreement cannot be provided until FY 16, each price proposal should include a price for the first year of service as a separate line item. 3) Additional questions and answers are attached to the February amendment.
