INTRODUCTION This is a pre-solicitation non-competitive notice of intent synopsis to award a contract without providing for full or open competition. THIS IS A NOTICE OF INTENT, NOT A REQUEST FOR QUOTE. A SOLICITATION WILL NOT BE ISSUED AND QUOTES WILL NOT BE REQUESTED. The National Institute on Drug Abuse (NIDA), Consolidated Station Support and Simplified Acquisitions (CSS/SA) Branch intends to negotiate and award a purchase order to MedStar Georgetown University Hospital for specialized psychiatric services. NORTH AMERICAN INDUSTRY CLASSIFICATION SYSTEM (NAICS) CODE This procurement is classified under 541990 - All Other Professional, Scientific, and Technical Services with a Size Standard of $15.0 Million. REGULATORY AUTHORITY The resultant purchase order will include all applicable provisions and clauses in effect through the Federal Acquisition Circular 2005-78, effective 26 Dec 2014. This acquisition is for a non-commercial item or service and is conducted under the procedures as prescribed in FAR Part 13-Simplified Acquisition Procedures. This acquisition is NOT expected to exceed the simplified acquisition threshold. STATUTORY AUTHORITY This acquisition is conducted under the authority of the authority of FAR Subpart 13.106-1(b)(1)(i). In addition, the full text of a clause may be accessed electronically at this/these address(es): FAR Clauses: http://acquisition.gov/far/ BACKGROUND AND PURPOSE In 2008, the District of Columbia Department of Health and NIH, including staff from the Clinical Center and the NIMH, launched a new partnership to make D.C. a leader in the response to the HIV/AIDS epidemic. This is referred to as the DC-PFAP (DC Partnership for AIDS Progress). For the first time, the nation's capital and a leading health research institution joined together to work with the District's universities and community-based health care providers to bring new ideas, services, and access to clinical research to D.C. residents. The partnership draws upon a diverse portfolio of academic institutions, community-based organizations, and stakeholder groups for the design and implementation of specific projects and activities. The D.C. Partnership for AIDS Progress will develop a cohort of patients to do long term assessment of neurocognitive function (the Neurocognitive Initiative). The work will involve recruiting patients from clinics in the District of Columbia, and enrolling them in a comprehensive assessment protocol. These patients will also be offered opportunities to participate in additional studies including, but not limited to, MRI imaging, PET scans, lumbar punctures, virologic studies, immunologic studies, and neurocognitive testing. Some of this work will be done at DC clinics, and some at NIH. A parallel cohort of active and retired military will be assessed as well if regulatory issues can be satisfied. PROJECT DESCRIPTION The purpose of this requirement is to provide the District of Columbia Partnership for HIV/AIDS Progress (DC-PFAP) with specialized psychiatric services for the purpose of working with psychiatric patients in D.C. clinics providing psychiatric services and health support to D.C. clinics that are designated clinical sites for the Neurocognitive Initiative protocol. The contractor shall provide a board-certified psychiatrist to provide standard of care psychiatric services for patients designated by the program. All services shall be provided under the direction of Dr. Maryland Pao. The work will include: •Assisting with the development of clinical projects in collaboration with the Neurocognitive Initiative and the DC-PFAP program leads •Monitoring development and assisting with the execution of specific clinical projects as assigned by program leadership •Under the supervision of NIMH Program Leadership, will establish communications with project stakeholders about goals, priorities, performance, and results of projects; stakeholders will include D.C. Department of Health officials, NIH leadership, patient care providers; patient groups, and community and civic groups •Screening patients to determine eligibility to participate in the Neurocognitive Initiative protocol •Analyzing clinical project data and preparing data for administrative presentation and scientific publication and presentation The contractor shall see all selected patients at the designated clinics who have been referred by the primary care provider. Patient participants shall be screened to determine eligibility for participation in a Neurocognitive Initiative protocol. Specifically, the physician shall: • Assure delivery of appropriate psychiatric care •Assess patients for enrollment in NIH-supported protocols •Assist with the development of research projects that will enhance NIH research in HIV/AIDS •Increase minority participation in research by assisting with the development of infrastructure needed to ensure access to participation in NIH research studies; establishing awareness about the importance of NIH research with medical and non-medical staff and with patients; and assisting with the implementation of systems for identifying and referring patients •Assist with the development of processes and procedures for clinical data collection •Assist with establishing infrastructure and mechanisms for coordination of programs KEY PERSONNEL A physician shall be selected by contractor for this specialized requirement. Necessary requirements for the selected physician include: •Board certification in psychiatry •Board eligibility in psychosomatic medicine •Experience in working with HIV-infected patients with psychiatric or neurocognitive co-morbidities •Experience in conducting clinical research PERIOD OF PERFORMANCE The anticipated period of performance is 2/20/2015 through 2/19/2016. A firm fixed price purchase order is contemplated. CONTRACTING WITHOUT PROVIDING FOR FULL OR OPEN COMPETITION (INCLUDING BRAND-NAME) DETERMINATION Georgetown University Hospital, under the auspices of MedStar Georgetown Medical Center, is a premier medical research institution. Market research has revealed that Georgetown University Hospital Department of Psychiatry has faculty members with unique expertise in Washington, D.C., which includes a strong background in psychosomatic medicine. This background is critical for providing excellent clinical services to this challenging patient population with high prevalence of HIV/AIDS and other chronic medical conditions, in addition to psychiatric co-morbidities. Market research performed concludes that there are currently no other medical teaching universities that can provide the required staff specializing in psychosomatic medicine. In particular, the market research revealed that there are zero physicians located at medical teaching universities that hold a board certification in psychiatry, have board eligibility in psychosomatic medicine, experience in working with HIV-infected patients with psychiatric or neurocognitive co-morbidities, and experience in conducting clinical research. In addition, a Master Affiliation Agreement was signed on July 1, 2014 between MedStar Georgetown University Hospital and the National Institute of Mental Health whereas the parties can, by a training affiliation, can complement its own facilities for the purpose of graduate medical education. CLOSING STATEMENT THIS SYNOPSIS IS NOT A REQUEST FOR COMPETITIVE QUOTES. However, interested parties may identify their interest and capability to respond to this notice. Responses to this notice shall contain sufficient information to establish the interested parties' bona-fide capabilities for fulfilling the requirement and include: unit price, list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. All offerors must have an active registration in the System for Award Management (SAM) www.sam.gov." A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. The information received will normally be considered solely for the purposes of determining whether to proceed on a non-competitive basis or to conduct a competitive procurement.
