- Questionnaire THERE IS NO SOLICITATION AVAILABLE AT THIS TIME. REQUESTS FOR A SOLICITATION WILL NOT RECEIVE A RESPONSE. This Request for Information is in support of Market Research being conducted by the Defense Commissary Agency to identify companies that provide full service nutritional guide programs that can support the DoD's Heathy Base initiative. Program objectives for this draft requirement: -Provide registered Dietician consultation • Provide guidance and advice on operating a Nutrition Guide Program -Compare commercial off the shelf database to resale items sold at military commissaries • Database attributes shall be in strict accordance with USDA Dietary Guidelines and FDA nutrient health standards. • Database must have the ability to be commissary specific or have the capability to identify which items apply to each commissary. • Must conduct initial and monthly database comparisons to identify new items, changes, and deletions. -Design a custom nutrition tag unique to DeCA • Tag will adhere to the electronic shelving labels • Display 2-3 most applicable attributes by food category and adjust attribute icon to fill tag's white space • Water/moisture resistant • Repositionable • Must adhere in frozen temperatures for at least 6 months • Non-curling • Free of adhesive residue when removed • Be 1¾ inch wide x 3½ inches high • Must have a transparent top (adhesive part) • Full color • Packaged and shipped by store and then separated by department, commodity, generic or planogram sequence -Stand-alone value tags • Tag will stick to the electronic shelving labels • Water/moisture resistant • Repositionable • Must adhere in frozen temperatures for at ;east 6 months • Non-curling • Free of adhesive residue when removed • Be 1¾ inch wide x 3½ inches high • Must have a transparent top (adhesive part) • Orange Color 1655c background with the word "VALUE" written on it with white Folio BT font The government requests that parties interested in this requirement provide a filled out questionnaire below and return it to Michael Barnes at michael.barnes@deca.mil. Responses are due back no later than Thursday, February 19, 2015.
