STATEMENT OF WORK For Northern Arizona VA Healthcare System (NAVAHCS) Building 161, Mental Health Outpatient Clinic Name Brand or Equal Meet or Exceed Specified Partition FOB Destination Delivery and Installation Must Occur Before April 1, 2015 1.0PROJECT DESCRIPTION: Provide all qualified labor, materials, tools, and supervision for the direct installation of a partition wall between Rooms 241A and 243A in Building 161 at the Northern Arizona VA Healthcare System (NAVAHCS) in Prescott, Arizona. This wall is an operable partition located in the new Mental Health Building 161. 2.0TASKS The complete installation of an operable panel between Rooms 241A and 243A in Building 161 at the NAVAHCS, Prescott, AZ. "Contractor shall field verify all dimensions "Complete placement of operable wall storage closet double doors (Two (2) sets) "Complete prep/patch and painting of operable wall closet "Must follow touchup and repair any damage to adjacent walls, floor and ceiling and return to original condition prior to installation "Prior to project start, please contact Randall S. Fenton, CFM, Senior Projects Technician at (928) 848-6841, Randall.fenton@va.gov 6.0SUBMITTALS 6.1Summary: This section specifies requirements for the submittal process. 6.2Product data for partition 6.3General Procedures: Unless noted otherwise, the Contractor is required to submit all shop drawings and material submittals for the contract within 10 working days after issue of the Notice of Award or as directed by the Contracting Officer. Data shall be collected into a single submittal for each element. Submit a minimum of 2 copies of all data for each submittal. 6.4Prior to start or to start of any work, the contractor shall submit the following: 6.4.1Schedule of work 6.4.2Detailed schedule of values cost breakdown in accordance with the general outline of work as detailed above 6.4.3Safety Plan to include Risk Assessment to the VA's patients and staff. 7.0QUALITY ASSURANCE 7.1All permits and/or notifications required by the governing agencies shall be the responsibility of the contractor. 8.0WORK ACCESS: This contract's installation worker will have access as coordinated with the Senior Projects Officer, Randall S. Fenton 9.0WARRANTY: The warranty will be in effect for one calendar year period from owner acceptance. Warranty includes parts, labor, and software reloads.
