Description

Statement of Work (SOW) Feeding Tube Real Time Tip Tracking System A. Background: The Southern Arizona VA Health Care System (SAVAHCS) located at 3601 S. 6th Ave,, Tucson, AZ 85723, has a requirement to replace the current feeding tube real time tip tracking system. Due to advances in technology the current operational system is being upgraded by the Corpak Medsystems, Inc. and the disposable supplies and older equipment will no longer be available. The upgraded operational systems affords SAVAHCS the ability to provide a safer placement, with real-time bedside confirmation of correct placement and reduced patient exposure to X-ray and potential infection if transported to X-ray. Additionally, these newer upgraded systems are proven to be more economical for hospitals with dramatically reduced need for X-rays, fewer complications or adverse events. B. Requirements: The Southern Arizona VA Health Care System (SAVAHCS) requires the following: "An electromagnetic transmitting stylet and feeding tube real time tip tracking system that generates a real-time signal as the feeding tube is inserted and advanced to the desired placement. "A Smart Receiver Unit (SRU) that receives the signal from the transmitting stylet as it is tracked through the placement procedure. "All -in-one monitor with integrated visual display terminal (VDT) that is touch screen and embedded computing system. The all-in-one monitor triangulates the signal from the SRU and displays a real-time representation of the feeding tubes tip's passage as it proceeds down the esophagus and into the preferred placement position. "Disposable Enteral Access System Electromagnetic Transmitting Stylet and Ultra-Lite Feeding Tube. CORTRAK 2 EAS (CORTRAK 2 Enteral Access System): CORTRAK 2 EAS Monitor - CORTRAK 2 EAS Receiver Unit - Stylet Interconnect Cable Operator's Guide - USB Flash Drive - External Power Supply - Receiver Unit Stabilizer Calibration Stylus - 2.5mm Allen Wrench - Warranty Transformer Supply Bracket Kit for CORTRAK Stand CORTRAK 2 EAS Feeding Tubes: 8 French, 36" Length - 10 French, 36" Length - 8 French, 43" Length - 10 French, 43" Length 12 French, 43" Length - 8 French, 55" Length - 10 French, 55" Length Annual Maintenance/Service Agreement (as needed) Any additional items as needed or required. ? C. Contract Administration Data: The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. Only the Contracting Officer is authorized to make commitments or issue changes that will affect price, quantity or quality of performance of this contract. The Contractor shall: "Communicate with the Contracting Officer on all matters pertaining to contract administration via email or phone. "Direct all questions and problems regarding this contract in writing to the Contracting Officer. "The contractor shall submit timely and correct invoices.
