- The Department of Veterans Affairs (VA) Wilkes Barre, PA Medical Center intends to negotiate on a Sole Source basis with Baxter Healthcare Corporation, One Baxter Parkway, Deerfield, IL 60015-4633, to award a Six (6) month Base, and Two one-year Option Year's Contract, to furnish the following Gambro (Baxter Healthcare Corporation) Hemodialysis supplies, in accordance with the attached Statement of Need. Estimated Base and Option Year Price - $185,988. Statement of Need: Hemodialysis Supplies are needed for the Gambro Phoenix Dialysis Machines used for end stage renal disease patient care requiring dialysis. These machines are proprietary to Baxter Healthcare and the supplies needed must be compatible with the Gambro Phoenix Dialysis Machines. A base year contract with 2 option years is required to begin 1/6/15. CASES NEED TO BE DELIVERED BY THE FIRST TUESDAY OF EACH MONTH FOR A PERIOD OF ONE YEAR.
