Description
CHILDCARE ADDENDUM TEMPORARY WAIVER FORM This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotes (RFQ) #246-15-Q-0009. This procurement is a 100% small business set-aside under NAICS code 621111 with a standard business size of $11.0 million. Contractor shall provide a firm-fixed rate for twenty-four (24) eight (8) hour clinics, for a one year period of service. Estimated start date is April 1 2015. The closing date for receipt of quotes is March 12 2015 at 4:30 pm. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on March 12 2015. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Please do not email RFQ's. Qualified vendors will review the following and submit applicable information. No phone inquiries! STATEMENT OF WORK - CONTRACT NEPHROLOGIST I. STATEMENT OF WORK: The Nephrology Clinic will operate as a physician referral-based clinic serving the Wewoka Service Unit area. The contractor will serve as a Nephrologist Consultant to the Wewoka Service Unit. The contractor will provide initial and ongoing care to the IHS renal patients. The contractor will also be available on his clinic days for telephone consultation with outlying IHS clinics and physicians in the Wewoka Service Unit area. Service shall include, but not be limited to the following: A. Provide nephrology services to patients on-site at the Wewoka Indian Health Center, Wewoka, Oklahoma for the number of hours stated during specified time. B. Number of patients to be seen and when services are needed will be determined by the Wewoka Indian Health Center Clinical Director. C. The Contractor shall examine, diagnose, and develop treatment plans and recommendations for patients seen. D. The Contractor shall recommend and refer patients to other Indian Health Service providers and CHS vendors within appropriate IHS guidelines. E. The Contractor will document services, education and treatment of patients in the adopted template for nephrology on the Electronic Health Record (EHR). F. The Contractor will utilize the Electronic Health Record (EHR) notification systems, to communicate with the nursing and medical providers at the Wewoka Indian Health Clinic. The notification system is set up to provide a continuity of care for the patients. G. The Contractor will be responsible for actions necessary for TJC standard compliance and administrative policies as directed. H. The Government will provide the facilities, office, equipment and supplies for use by the Contractor. I. The Contractor shall provide consultative reports to be done at the time of examinations in the EHR, furnished by the Government. J. The Government will provide assistance and orientation and guidance in seeing patients as well as receiving supplies, utilizing and ordering laboratory, diagnostic exams, and pharmacy services. K. The contractor will order medications utilizing the EHR in accordance with the Wewoka Indian Health Clinic Formulary and its restrictions thereof. II. PROFESSIONAL QUALIFICATIONS: A. The Contractor shall provide physician(s) who meet(s) the following minimum qualifications and requirements: 1. Current unrestricted license to practice medicine in the United States. 2. The Contractor is responsible for obtaining that amount of professional liability insurance coverage of at least $1,000,000 per occurrence/$3,000,000 aggregate. 3. Physician(s) must have completed a residency program in an approved Nephrology Residence or Board Certification in Nephrology. This category may serve as consultants in Nephrology. 4. Physician(s) provided by the Contractor must apply and be accepted as Associate members of the Medical Staff, Wewoka Indian Health Center. B. Evidence of 1, 2, and 3 above must be provided to the Contracting Officer prior to award of contract, however, it is recommended that credentials (copies) accompany the Request for Quotations in order to shorten evaluation time. C. In providing such service Contractor will comply with policies, procedures and by-laws of the Wewoka Indian Health Center. Contractor will also be required to perform this service in compliance with applicable standards of the Joint Commission and of quality of care. III. INDEMNITY AND INSURANCE: The Government assumes no responsibility for negligent acts of the Contractor; therefore, the Contractor is responsible for obtaining that amount of professional liability insurance coverage of at least $1,000,000 per occurrence/$3,000,000 aggregate. The Contractor will keep harmless and indemnify the Government against any or all loss, cost, damage, expense or liability whatsoever as a result from the performance of the Contractor. Contractor will hold the Federal Government and its agents including Clinical Director and the governing body immune from civil or professional liability for all acts related to quality care management and enforcement of this contract. IV. GOVERNMENT FURNISHED FACILITIES, EQUIPMENT AND SUPPLIES: The Government will provide the facilities of the Wewoka IHS Health Center, examination room, for use by the Contractor in the provision of Nephrology Service. V. WORK SCHEDULE: A. The Contractor shall provide Nephrology clinics onsite at the Wewoka Indian Health Clinic. B. The Contractor shall provide a minimum of two eight-hour nephrology clinics per month in the Wewoka Indian Health Center outpatient department and will conduct group teaching sessions for patients and in-service education for staff. These clinics will run from 9:00 a.m. to 4:00 p.m. on Nephrology clinic days, starting approximately April 1 2015 through March 31 2016. C. Any changes to the preceding shall require mutual agreement of the Contractor and the Contracting Officer's representative. VI. REPORTING REQUIREMENTS: A. Reporting requirements shall include recording of all pertinent patient care information required in performance of Nephrology services as set forth in the medical by-laws of the Wewoka Indian Health Center. B. The Contractor shall submit an original and two (2) copies of this invoice to: Oklahoma City Area Indian Health Service Division of Financial Management 701 Market Drive Oklahoma City, OK 73114 C. Each invoice shall include the following information: 1. Contractor's name and invoice date. 2. Contract number or other authorization for delivery of services. 3. Description, price and quantity of services actually rendered. 4. Name, title, phone numbers and complete mailing address of official to whom the payment is to be sent. 5. Invoice shall be submitted no later than ten (10) calendar days following the last day of each month. VII. REFERENCE MATERIALS/OTHER FACTORS: Contractor shall be required to possess familiarity and abide by the current policies, by-laws and regulations of clinical and medical staff of the Wewoka Indian Health Center. The Government intends to award one from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Price; (2) Qualifications. Both factors are equal in importance.
