- DRAFT PWS 1) Type of Notification: Sources Sought (FBO) This is a sources sought synopsis only, (not a pre-solicitation notice pursuant to FAR Part 5). The purpose of this notice is to gain knowledge of potential qualified sources. Responses to this notice shall include the following: (a) company name (b) address, (c) point of contact, (d) "number of employees" as defined by the Federal Acquisition Regulation (FAR) part 19.101 and (e) the amount of "annual receipts" as defined by the FAR part 19 (f) Business size. In addition, the respondent must provide a capability statement that addresses the organization's qualifications and ability to perform as a prime contractor the work described below. Please see the attached DRAFT Performance Work Statement for a full list of duties and requirements. 2) Description: Personal Services-Surgical Technician (4 FTEs) The following is a brief summary of the primary duties that the Contractor must perform: The Contractor shall provide personal services for personal services for Surgical Technicians at San Antonio Texas Department of Defense Military Treatment Facilities (MTF). The CP shall perform services and support compatible with the medical facility's operating capacity and equipment. 3) Hours of Performance: The Contract Personnel (CP) shall provide personal services for Surgical Technicians at San Antonio Texas Department of Defense Military Treatment Facilities (MTF). The CP shall perform services and support compatible with the medical facility's operating capacity and equipment. 4) Qualifications: 4.1 The CP shall meet the following education requirements: The CP shall have a high school education or GED equivalent and must have successfully completed an accredited surgical technician course. 4.2 The CP shall have and maintain a current Basic Life Support (BLS) certification. 4.3 The CP shall meet the following experience requirements: Must have at least three (3) years of experience within the last five (5) years as a surgical technician. Shall have surgical technician experience in ophthalmology, otolaryngology, orthopedic, general, oral, and gynecology surgical specialties. 5) Place of Performance: These services will be performed at San Antonio Texas Department of Defense Military Treatment Facilities (MTF). This survey is to determine the capability of potential sources and the availability of commercial sources and services pursuant to FAR Part 10. This notice is for informational purposes only and does not constitute an Invitation for Bid or Request for Proposal, and is not to be construed as a commitment by the Government. The Government will not reimburse contractors for any cost incurred for their participation in this survey. Responses should contain Company Name, Address, Point of Contact, Cage Code, Phone Number, FAX Number, Size of Business pursuant to NAICS code 621399 (business size standard $7,500,000.00), a brief resume of performance of similar services of the magnitude described above (include contact and phone number of projects listed), a statement as to whether your company is domestic or foreign owned. Responses must be received no later than 3:00 p.m. CST on 4 March 2015. Prospective vendors are cautioned that if you cannot provide Personal Services for Surgical Technicians, please do not respond to this notice. All prospective offerors who may be interested in participating in a future procurement for this requirement should respond to this Source Sought Notice by responding in FBO's electronic system or by contacting Dawn Gayden at dawn.gayden@us.af.mil by the above listed response date. Please include the company's SBA size status for the listed NAICS Code and the company's socioeconomic status, if any. In the event a prospective vendor is selected for an award, the selected vendor must be registered in the Systems for Awards Management website at www.sam.gov.
