- DESCRIPTION: This is a combined synopsis solicitation for a commercial item prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented by additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. Proposals are being requested for Request for Proposal (RFQ), VA528-15-Q-0126 for the following: Replacement of damaged Terra Cotta at the Canandaigua VA Medical Center (see attached SOW) Solicitation VA528-15-Q-0126 is issued by the Canandaigua V.A. Medical Center, 400 Fort Hill Ave, Canandaigua, NY 14424 who anticipates not conducting discussions with offerors submitting quotations under the subject solicitation and reserves the right to make contract award based on the initial offer submitted. Requirements may be added by future amendments to this solicitation. The Government contemplates award of a Firm Fixed Price contract resulting from this solicitation. IAW FAR 19.5 Set-Asides for Small Business, this solicitation will be set aside to small businesses. SYNOPSIS/SOLICITATION Proposals must be received no later than 2:00 pm, EST on Wednesday March 13, 2015. Offers may be submitted via email to Sam DeMuzio, sam.demuzio@va.gov. When submitting offers, please note the solicitation number VA528-15-Q-0126 in subject line of the email. Please note that the offeror assumes all risk for any delay in the transmission of their proposals. Any offers received after the 2:00 PM EST deadline will not be considered. All offerors are encouraged to review FAR 52.212-1, INSTRUCTION TO OFFERORS-COMMERCIAL ITEMS and Internal Instruction, INT L2.11-4, E-MAIL QUOTATIONS and to submit their proposals early. R Statement of Work Architectural Terra Cotta A.GENERAL INFORMATION 1.Title of Project: Architectural Terra Cotta 2.Type of Contract Contemplated: An open market purchase of goods, Architectural Terra Cotta materials. B.SCOPE This is a Statement of Work for the acquisition of Architectural Terra Cotta materials for Building 9 of the Department of Veterans Affairs, Canandaigua VAMC. The project is being performed at the main facility (Bldg. 9) located at 400 Fort Hill Ave; Canandaigua, NY, 14424. The project consists of purchase and receiving of Architectural Terra Cotta Materials: "Glaze color match "Shop "3 models "3 molds "1 Terra Cotta unit-Coping peak with double return "1 Terra Cotta unit- Coping transition to peak, Left "1 Terra Cotta unit- Coping transition to peak, Right C.REQUIREMENT/SALIENT CHARACTERISTICS/SPECIFICATIONS Common Nomenclature: Architectural Terra Cotta for cladding and restoration of building façade Kind of material: Clay formula B21 Tan, high strength fired ceramic. No alternatives available that surpass terra cotta in longevity, aesthetics and sustainability, as well as its ability to withstand severe climates both for freeze-thaw and UV resistance. Terra cotta is the historically accurate façade material, and meets Historical Preservation requirements. Electrical Data: N/A Dimensions, size, capacity: 30" x 16", 30" x 16", 26" x 50" finished face unfolded dimensions Principles of operation: The Contractor shall submit an estimated dock delivery schedule. The Contractor shall be responsible for any damage to the materials that occur due to Contractor/freight error or neglect. The Contractor will be responsible for the coordination for delivery and receiving to the Canandaigua VAMC. The Contractor will perform a field visit to pick up samples. Price includes (3) submittals only. Additional charges will apply for if more than (3) submittals are required. Shop Drawings. The Contractor will exercise use of current independent ASTM C67 lab test results. All other costs associated with lab testing to be borne by others. Materials must match color, size, and composition of current building structure due to historical site preservation. Restrictive environmental conditions: Intended use: Architectural Terra Cotta for cladding and restoration of building façade Equipment with which the item is used: Custom made models and molds produced 8% larger than historic samples. Hand pressed, hand finished, and hand applied custom color matched glaze. Kiln fired to over 2000 degrees Fahrenheit. New terra cotta units shrink 8% during production process and will closely match historical units in dimension and color after kiln firing. End item application: Coping Original equipment manufacturer part number: (1) coping peak with double return; (1) coping transition to peak Left; (1) coping transition to peak Right. Other pertinent information that describes the item, material or service required: (See attached Technical Data Sheet for Architectural Terra Cotta) D.OTHER UNIQUE REQUIREMENTS Discuss any other unique requirements or considerations, e.g. Compatibility with Existing Equipment: Warranty: Seller must warrant the manufactured Terra Cotta products against any defects in workmanship and materials for a period of 5 years. E.DELIVERY The items must be delivered to: Canandaigua VAMC Att: Scott Anderson 400 Fort Hill Avenue Canandaigua, NY 14424 Building 9 Facility will be available for deliveries between the hours of 0800 and 1600 M - F excluding Federal Holidays. F.Government-Furnished Property, Material, Equipment, or Information (GFP, GFM, GFE, or GFI) N/A G.SECURITY Delivery must notify the VA Engineering department upon arrival, prior to delivering the materials. H.SPECIAL MATERIAL REQUIREMENT Materials must match color, size, and composition of current building structure due to historical site preservation. I. PLACE OF PERFORMANCE Canandaigua VAMC 400 Fort Hill Avenue Canandaigua, NY 14424 Building 9 Facility will be available for deliveries between the hours of 0800 and 1600 M - F excluding Federal Holidays.
