Description
This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in the Federal Acquisition Regulations (FAR) Subpart 12 - Acquisition of Commercial Items and FAR Subpart 13 “ Simplified Acquisition Procedures, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on both FEDBIZOPPS and NECO (http://www.neco.navy.mil/). The RFQ number is N00604-15-T-3042. This solicitation document incorporates provisions and clauses in effect through FAC 2005-78 and DFARS Publication Notice 20150226. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The NAICS code is 532490 and the Small Business Standard is $32.5M. The proposed contract is 100% small business set-aside. The Small Business Office concurs with the decision. NAVSUP Fleet Logistics Center Pearl Harbor Regional Contracting Department requests responses from qualified sources capable of providing the following: CLINQTY U/I 0001 3 Each 3-Ton Diesel Powered with tines (forks) of 6 ft in length with Lifting Brackets Lifting brackets that are able to support weight of forklift, used to lift and place forklift into dry dock with crane. The lease duration is 23 March 2015 through 30 September 2015. 0002 1 Each 7.5-Ton Diesel Powered with tines (forks) of 6 ft in length with Lifting Brackets Lifting brackets that are able to support weight of forklift, used to lift and place forklift into dry- dock with crane. The lease duration is 23 March 2015 through 30 September 2015. Provide operator ™s manual to PHNSY and IMF. Provide all safety bulletins including past ones when the operator ™s manual is delivered and any other bulletins immediately as they are issued. Provide a copy of the certification of the personnel lift. Provide any training videos, if available. Provide instructions for any unusual pre-operation safety checks that need to be completed. The government will not be responsible for the condition of the equipment after the expiration of the Period of Performance. The forklift shall be delivered on the first day and picked up on the last of the day Period of Performance to PHNSY and IMF. The government will not be responsible for the condition of the equipment after the expiration of the Period of Performance. Forklift Extensions - Use of Forklift Extensions must comply with all OSHA Standards/Rules/Guidelines. The OEM must specifically submit approval for the use of the Forklift Extensions for the specific make/model of the forklift. The Manufacturer must send a new label plate to be installed on each individual forklift where specific Forklift Extensions will be used, which will annotate the new load capacity, load centers, and all other information which will be specific to the use of the forklift with the specified forklift extensions. The label plate must come from the Manufacturer.
