HU0001-15-Q-0404 Provisions and Clauses (P&C) HU0001-15-Q-0404 Required Provisions (Required) HU0001-15-Q-0404 CLIN Structure (Required) HU0001-15-Q-0404 Request for Quote (RFQ) *Refer to the attachments for the complete detail General Overview: The Uniformed Services University of the Health Sciences (USU), located in Bethesda, MD, has a requirement for an annual PsychiatryOnline DSM Premium Subscription renewal. The USU is the only Federal health sciences university and we are committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense (DoD) and the United States Public Health Service (PHS) and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USU is located in Bethesda, MD on the grounds of the Naval Support Activity Bethesda. Additional information regarding USU and its mission can be found on our website: www.usuhs.mil. This is a combined synopsis/solicitation for commercial items in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 Acquisition of Commercial Items and Part 13 Simplified Acquisition Procedures. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular FAC 2005-80, effective March 2, 2015 and Defense Federal Acquisition Regulation Supplement (DFARS), effective February 26, 2015. This requirement is set-aside for small business concerns under North American Industry Classification System (NAICS) Code 519130 Internet Publishing and Broadcasting and Web Search Portals with a size standard of 500 employees. In order to be eligible for award, offerors must be registered on SAM.gov with completed Representations & Certifications (Reps & Certs). Offerors not registered on SAM.gov with completed Reps & Certs will not be eligible for award. Offerors interested in responding to this requirement must provide a quote for an annual PsychiatryOnline DSM Premium Subscription renewal, which at a minimum, meets the specifications listed below. A single firm fixed-priced (FFP) purchase order will be awarded. Specifications: USU, which falls under Tier 3, requires an annual, unlimited institutional (site) license for the PsychiatryOnline DSM Premium Subscription without any concurrent usage restrictions. The USU users must be able to access the journals online at any time of their convenience without any restriction. Because we're seeking an institutional (site) license, the number of users, number of sites, and the locations should not be pertinent. The period of performance (PoP) is from April 1, 2015 to March 31, 2016. The subscription renewal must include: 15 reference books including: 1.American Psychiatric Association (APA) Practice Guidelines in both comprehensive- and quick-reference formats 2.DSM-5 3.DSM-5 Clinical Cases 4.DSM-5 Handbook of Differential Diagnosis 5.Dulcan's Textbook of Child and Adolescent Psychiatry 6.Gabbard's Treatments of Psychiatric Disorders 7.Helping Parents, Youth, and Teachers Understand Medications for Behavioral and Emotional Problems 8.Manual of Clinical Psychopharmacology 9.Textbook of Psychotherapeutic Treatments 10.Textbook of Traumatic Brain Injury, Second Edition 11.The American Psychiatric Publishing Textbook of Geriatric Psychiatry, Fourth Edition 12.The American Psychiatric Publishing Textbook of Psychiatry, Sixth Edition 13.The American Psychiatric Publishing Textbook of Psychopharmacology, Fourth Edition 14.The American Psychiatric Publishing Textbook of Substance Abuse Treatment, Fourth Edition 15.What your Patients Need to Know About Psychiatric Medications Four peer-reviewed journals (access includes 1997-Present): 1.American Journal of Psychiatry 2.Journal of Neuropsychiatry and Clinical Neurosciences 3.Psychiatric Services 4.Psychiatric News Your subscription also includes access to: 1.The complete DSM archives (DSM-I, DSM-II, DSM-III, DSM-III-R, DSM-IV, and a couple of significant changes to between-edition printings) for research and archiving purposes 2.A Best-Selling Title from APP each month for 12 months (PDF format) PsychiatryOnline's institutional license is specially designed for meet the user community needs: 1.Sophisticated semantic indexing based on the National Library of Medicine's Unified Medical Language System (UMLS) to yield more relevant hits and to direct users to related content from wherever they are in the site 2.Remote access for authorized users 3.No concurrent usage restrictions 4.COUNTER-compliant usage statistics 5.Library branding 6.Ability to email full text of individual sections to colleagues 7.Regular updating Offerors responding to this requirement must provide pricing, at a minimum, for the following contract line item numbers (CLINs): CLINDescriptionQuantityUnit of Issue 0001PsychiatryOnline DSM Premium Subscription Renewal1Year The PoP is from April 1, 2015 to March 31, 2016. In order to be considered for award, offerors must provide a quote for all CLINs listed. Evaluation / Award Detail: The Government intends to award a single FFP purchase order on the basis of Lowest-Price Technically-Acceptable (LPTA) source selection process. Offerors must submit a copy of the completed CLIN Structure spreadsheet* in the quote package. Please note there are two sheets to be completed in the Excel file. Offeror must inform their preferred method of payment (either DFAS EFT or GPC Visa). The Federal Government prefers not to use third-party-payment merchants, e.g. PayPal, to exchange currencies. This is because these merchants can mask the actual merchant/vendor and impede the Government's effort to promote transparency. If these third-party-payment merchants must be used, offeror must include a compelling justification in the quote package. This will be considered as a part of the Price evaluation factor. See attached list for applicable provisions and clauses for this requirement. Offerors must submit a copy of completed Required Provisions. Failure to do so may result in the quote package being deemed nonresponsive. The Government reserves the right to award without discussions. Questions/clarifications regarding this solicitation must be submitted via email to andrew.lee@usuhs.edu by 2:00PM ET on March 13, 2015. Any question received after 2:00PM ET on March 13, 2015 will not be answered. Questions will not be answered over the phone or submitted by any other means. All questions will be answered in an amendment to the solicitation posted on or around March 16, 2015. Quote packages are due by 10:00PM ET on March 19, 2015. Quotes must be submitted via email to andrew.lee@usuhs.edu. Quotes will not be accepted by any other means. Late quotes will not be considered. Quote packages should include a cover sheet that provides the following information: a.Official Company Name; b.Point-of-Contact, including name, email address, and phone number; c.DUNS Number; and d.Statement that includes acknowledgement of latest amendment number-only if an amendment had been issued for this RFQ.
