Description
This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number is VA257-15-Q-0691. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-75, effective June 24, 2014. The associated North American Industrial Classification System Code (NAICS) for this procurement is 811219 with a small business standard of 20.5 Million Dollars. This acquisition is solicited as a set-aside for small business. Network Contracting Office 17 requests responses from qualified sources capable of providing the following item(s): Patient Lift Inspections at the Temple and Waco VA Facilities Requirements The contractor shall be responsible for the Annual Inspection of Guldmann Patient Lifts due in April and Arjo Patient Lifts due in May at VA facilities in Waco, TX and Temple, TX. The work consists but not limited to: Weight testing each individual lift with appropriate weight. ** Annual inspections; which includes weight testing; must be done in the months of April and May of the calendar year. All inspections will be done on Central Texas Campuses, 4800 Memorial Drive, Waco, Texas and 1901 Veterans Memorial Blvd., Temple Texas. The contractor shall provide all; labor, tools, equipment and transportation to and from the different facilities to perform the work need to get the lifts inspected. The contractor shall be familiar with all of the terms, conditions, and requirements for annual certification of patent lifts. **Must provide proof of certification of personnel for Inspections/ weight testing Guldmann and Arjo Patient lifts. Project Requirements "Inspections will consist of checking 272 lift/hoist motors. "Ensure all lifts are working properly and operating under good conditions. "Verify all software is up to date on the lifts/hoist. "Examine all rails, power cords and functionality of the entire patient lift system. "Report any deficiencies to the VA COR. "Give a copy of annual inspection reports to the VA COR. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
