- COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation ("combo" hereafter), which is expected to result in award of a pre-priced blanket purchase agreement (BPA) for commercial items, i.e. Neonatal Screening services at Aviano, AB, Italy. As described under the paragraph below titled "Requirement." The combo is prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This acquisition uses simplified acquisition procedures IAW FAR Subpart 13.5--Test Program for Certain Commercial Items. This announcement constitutes the only solicitation; proposals and price lists are being requested and a written solicitation will not be issued. The Government intends to award a firm-fixed price BPA pursuant to this solicitation to the offeror whose offer, conforming to the solicitation requirements, represents the best value to the Government, price and other factors considered. The Government intends to award without discussions and the Government reserves the right to make no award at all depending upon the quality of offers received. Calls under the subsequent BPA will be paid by Government Purchase Card (GPC). Awardee MUST HAVE an individual account with their bank for processing GPC (credit card) payments. Awardee must have the capability to accept the GPC credit card. A letter from the contractor's bank stating their account is capable of processing GPC payments is REQUIRED PRIOR to award. Solicitation Number: FA5682-15-Q-0003 NAICS Code: 923120 - Maternity and child health program administartion Requirement: Aviano AB requires Neonatal Screening services for the 31 st Medical Group, Aviano AB, in accordance to the standards in the Statement of Work (SOW) as well as all local and federal regulations. Description Price per Specimen Initial Screen Disorders Confirmed or Detected by DNA Analysis A BPA arrangement will involve the below listed requirements: •a. The BPA is written for a period of 5 years and during the perios the government is only obligated for actual purchases made under the BPA. Therfore, the BPA will not include a minimum or maximum dollar limitation during this five (5) year period. The Air Force is not obligated to make any award or call against this BPA. •b. The Contracting Officer must approve the price lists your company submits prior to establishing the BPA. Price lists are valid for a specific period of time (normally one year); i.e. 1 Mar 2015 through 29 Feb 2016. •c. All prospective awardees are required to register at the System for Award Management (SAM) and to maintain active registration during the life of the contract. SAM can be accessed at https://www.sam.gov. Offerors and contractors may obtain information on registration and annual confirmation requirements by calling 1-888-227-2423, or via the internet at https://www.sam.gov. Prior to BPA award your company must be registered with SAM. •d. All contractors must also have a DUNS number. The DUNS number is a nine-digit number assigned by Dun and Bradstreet Information Services. If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. A DUNS number will be provided immediately by telephone at no charge to the offeror. For information on obtaining a DUNS number, the offeror, if located within the United States, should call Dun and Bradstreet at 1-800-333-0505. The offeror should be prepared to provide the following information: (1) Company name. (2) Company address. (3) Company telephone number. (4) Line of Business. (5) Chief executive officer/key manager. (6) Date the company started. (7) Number of people employed by the company. (8) Company Affiliation. The government will award a BPA resulting fom this solicitation to the responsible offeror whose proposal conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The factors that shall be used to evaluate proposals are Technical, and Price, as expounded below. The Technical factor will be rated "acceptable" or "unacceptable" based using the rating and descriptions in the table below: Rating Acceptable Proposal clearly meets the requirements. Unacceptable Proposal does not clearly meet the requirements in the solicitation.
