- SELF CERTIFYING DOCUMENT PAST PERFORMANCE QUESTIONNAIRE APPLE BPA RFQ DOCUMENT RFQ DOCUMENT - MUST READ KEY MANAGEMENT PERSONNEL LIST ACQUISITION RISK QUESTIONS PRICING MATRIX The Federal Bureau of Investigation intends to establish a 1 year Blanket Purchase Agreement (BPA) for Apple products. Please see the attached document named APPLE BPA RFQ DOCUMENT # DJF-15-1200-PR-APPLE. PLEASE MAKE SURE TO VIEW ALL ATTACHED DOCUMENTS AND COMPLETE ALL REQUIRED DOCUMENTS. Vendor Shall Provide: 1. A firm fixed price quote for each item/complete pricing matrix (Pricing Matrix Attachment #1) 2. Proof of being a SAM.GOV registered small business 3. Proof of being a authorized Apple Reseller 4. Self Certifying Document (Attachment #5) 5. Past Performance Questionnaire (Attachment #4) 6. Key Personnel List (Attachment #3) 7. Acquisition Security Questions (Attachment #2) If a quoter fails to provide any one of the seven required items above they will be considered non-responsive and not considered for award.
