- Current Wage Determination for Ward county, ND. Request for Quote Financial Institution Reference Sheet - fill out and return with offer. Performance Information Form - fill out and return with offer. List of monthly tasks. List of equipment to be maintained. Performance Work Statement Description: (i) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. (ii) The solicitation number F3C3MM5086AW02 is being issued as a request for quotation. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-81. (iv) This acquisition is 100 percent set aside for small businesses. The North American Industry Classification System (NAICS) code is 811490 with a size standard not to exceed $7.5 million (U.S.). (v) CLIN 0001 The contractor shall provide personnel, management, materials, parts, labor, tools, transportation and equipment necessary to provide preventative and/or required maintenance of the fitness equipment at the McAdoo Fitness Center on Minot Air Force Base, North Dakota in accordance with attached statement of work and Equipment Maintenance Plan from 1 Jul 2015 - 30 Jun 2016. Period will be for one (1) year plus four (4) option years. CLIN 0002 - Estimated Vinyl Repair and Replacement Parts from 1 Jul 2015 - 30 Jun 2016 CLIN 1001 - Option year 1, provide preventative and/or required maintenance of fitness equipment in accordance with attached statement of work from 1 Jul 2016 - 30 Jun 2017. CLIN 1002 - Estimated Vinyl Repair and Replacement Parts from 1 Jul 2016 - 30 Jun 2017. CLIN 2001 - Option year 2, provide preventative and/or required maintenance of fitness equipment in accordance with attached statement of work from 1 Jul 2017 - 30 Jun 2018. CLIN 2002 - Estimated Vinyl Repair and Replacement Parts from 1 Jul 2017 - 30 Jun 2018. CLIN 3001 - Option year 3, provide preventative and/or required maintenance of fitness equipment in accordance with attached statement of work from 1 Jul 2018 - 30 Jun 2019. CLIN 3002 - Estimated Vinyl Repair and Replacement Parts from 1 Jul 2018 - 30 Jun 2019. CLIN 4001 - Option year 4, provide preventative and/or required maintenance of fitness equipment in accordance with attached statement of work from 1 Jul 2019 - 30 Jun 2020. CLIN 4002 - Estimated Vinyl Repair and Replacement Parts from 1 Jul 2019 - 30 Jun 2020. **See attached Request for Quotation. (vi) Equipment shall be maintained in accordance with (IAW) Original Equipment Manufacturer (OEM) specifications and procedures. This includes technical assistance, Preventive Maintenance Inspections (PMIs), installations, removals, programming, service call equipment repairs, reupholstering, troubleshooting, and alignment required to maintain item(s) referenced in Appendix 2. Equipment shall be maintained at intervals dictated by and in accordance with the OEM specifications. (vii) Place of performance is Minot AFB, ND 58705 and period of performance will be 1 Jul 2015 - 30 Jun 2020. (viii) The provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. (ix) The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. The government will award a firm-fixed price contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. Other factors considered will be the ability to demonstrate the capacity to fulfill this requirement in accordance with the attached performance work statement. Award shall be based on best value to the government. The evaluation procedure to be used for this procurement is price and price related factors. Technical Acceptance Criteria will be evaluated on a pass/fail basis. In order to be technically acceptable the Offeror must: a. FACTOR 1 - PAST PERFORMANCE: This factor is met if the Offeror has successfully completed two contracts with a minimum value of $10,000 annually and has been actively engaged as a contractor performing services as defined by the NAICS Code 811490 within the last five years. Complete attached Performance Information Form and Financial Institution Reference Sheet. b. FACTOR 1 - RECRUITMENT/HIRING - This subfactor is met when the Offeror provides a detailed method of recruiting, hiring, and retaining qualified personnel who will be able to perform the service identified in the Performance Work Statement (PWS). (x) The government advises Offerors to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. Offerors are also reminded to complete Offeror Representations and Certifications on-line at www.bpn.gov prior to submitting an offer. (xi) Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition.
