STATEMENT OF WORK - SIMPLE C in the CLC A. GENERAL GUIDANCE 1. BACKGROUND: SimpleC Companion is needed for implementation of the pilot study-SimpleC Companion in the CLC-and for implementation of the life review intervention for Veterans with dementia in the study, Evaluating Sleep in Veterans and Their Family Caregivers, or SLEEP-E Dyads (E-7249-W). SimpleC Companion in the CLC This project is a pilot study funded by the Center of Excellence for Visual and Neurocognitive Rehabilitation at the Atlanta VA Medical Center to test the feasibility of using Simple C Companion in the Atlanta VA's Community Living Center (Eagle's Nest). There are two components of this pilot: 1) creating individualized programs for five (5) CLC residents, 2) encourage community engagement within the dementia unit in the CLC by providing SimpleC Community in all of the common areas (as approved by CLC leadership) and in the bathing areas Evaluating Sleep in Veterans and Their Family Caregivers (SLEEP-E Dyads) This project is a Career Development Award evaluating sleep not only with Veterans, but also with their cohabitating caregivers. The sleep evaluations will occur in Veteran/caregiver's homes, rather than requiring the Veteran to conduct a sleep test in a fabricated hospital environment. The specific objectives of this study are accomplished in three phases: 1) an evaluation phase, 2) a development and preliminary intervention field testing phase, and 3) an intervention refinement, revision and re-testing phase. Simple C Companion is required for phase two (2) and phase three (3) as a type of intervention for Veterans who have been diagnosed with dementia or who have demonstrated one or more signs of mild cognitive impairment. Per the IRB approved protocol, Veteran/caregiver couples are stratified by dementia status. Half of the Veterans who demonstrate signs of mild cognitive impairment will receive Simple C Companion as a type of intervention to assist with challenging behaviors that often accompany Alzheimer's disease and related dementias. Data Management: All data (collected for the personalization process at the beginning of the research studies) must be encrypted via AES encryption during any data transfer. The data transfer required for the implementation of the intervention will be stored on secure server controlled by SimpleC. The portable devices only contain data necessary for the intervention. The portable devices will not be connected to the VA network. No contracting staff will be interfacing with the VA network. All contracting staff that interacts with research participants will need to be vetted by the VA in the WOC credentialing process and approved by IRB and VA R&D Committees (see Contractor Personnel Security Requirements) The contracting staff will have access to participants': names, addresses (including city, state, and zip code), date of visits, date of birth, phone numbers, and full face photographic images. 2. SCOPE: SimpleC Companion is needed for research participants enrolled in these studies who have been diagnosed with dementia or are showing signs of mild cognitive impairment because the application provides research staff the ability to safely and calmly interact with research participants who have dementia, thereby improving the interaction for the participant and Caregiver. Since most disruptive behaviors occur around care routines (ADLs such as toileting, bathing, etc.) this feature is critical to the fidelity and integrity of the intervention and the overall success of the SimpleC in the CLC and the SLEEP-E Dyads projects. For the SimpleC Companion in the CLC pilot project, one (1) SimpleC Community Connect, one (1) SimpleC Shower/Tub Room Station, and one (1) wireless broadcaster are also required to facilitate with bathing, engagement, and cognitive stimulation activities. 3. VENDOR/CONTRACTOR REQUIREMENTS The vendor/contractor shall ensure the following equipment specifications. A.Data collected for the individualization of the SimpleC Companion will be stored at the Atlanta VA Medical Center and collected in collaboration with VA staff who are IRB approved for each project. B.SimpleC Companion: will provide personalized audiovisual therapies unique to each user (5 residents in the CLC, 20 subjects in the SLEEP-E Dyads project) C.SimpleC Community Connect: will connect to a large screen TV in the CLC common areas to decrease social isolation and encourage communication with residents D.SimpleC Shower/Tub Room Station: will provide a background program playing to decrease agitation while completing bathing tasks 4. TASKS The contractor shall complete the delivery of all equipment items listed. In addition, the following maintenance requirements, technical support requirements, and training, set-up and installation must be met. A.Maintenance Requirements. For the duration of the one year starting September 1, 2015, the vendor/contractor must: "Replace any equipment that does not meet the needs listed in the SOW that cannot be remedied through technical support B.Technical Support Requirements. For the duration of one year starting September 1, 2015, the vendor/contractor must provide: "Technical support subscription for Simple C companion within the CLC and within the homes of research participants/caregivers enrolled in the SLEEP-E dyads study "Technical support subscription includes support: oWithin 100 minutes of request for services Monday-Friday from 9:00-17:00EST for issues that arise during home visits. oWithin 72 hours of request for services at all other times C.Training, Set-up & Install. The contractor will set-up and install the SimpleC Companion program. The devices the program will reside in will never be connected to the VA network. The contractors will not have access to any VA systems. The Contractor must: "Include on-site implementation for five (5) SimpleC Companion programs individualized for five (5) residents in the CLC "Set up SimpleC Community Connect in the common areas in the CLC approved by the CLC leadership "Set up a Shower Room device to assist with disruptive behaviors that occur during bathing (calm residents, encourage bathing, engage residents, etc.) "Include on-site implementation for twenty (20) care recipients who are a member of a research participant caregiving pair and who are enrolled in the SLEEP-E Dyads sleep study "Train the formal caregivers in the CLC to use SimpleC Companion with the individual residents and with all of the residents in the common areas "Train the informal caregivers who live with the research participant/care recipient to use SimpleC Companion with the individual with signs of cognitive impairment within their own home "Ensure updates to SimpleC Companion are incorporated into the program as needed "Confirm specifications of the devices and accessories 5. DELIVERABLES The specific deliverables include the following: 1.Hardware Requirements. Must include: a.One (1) Community Connect System approved by CLC leadership (in CLC only) i.Quad core media hub for android ii.Includes SimpleC player with proprietary content including games, reminders, and engagement activity software iii.One (1) HDMI cable per TV approved by CLC leadership to connect player with TV iv.One (1) remote control per TV approved by CLC leadership to control Community Connect b.One (1) Shower/Tub Room Device i.High definition, water-proof, touch screen to facilitate bathing events by decreasing agitation and making the event more enjoyable for the residents (in CLC only) c.One (1) Wireless Broadcaster i.Required to install periodic updates for the SimpleC Companion application (in CLC only) ii.Wireless coverage is needed to span the greater Atlanta Area - 75 mile radius within research participant/care giver homes and within the Community Living Center (or Eagle's Nest) within the Atlanta VA Medical Center 2.Personalization Requirements. Must include for each of five (5) residents in the CLC and each of twenty (20) subjects in the SLEEP-E Dyads sleep study: a.Interview research participants to collect information on the following surveys (all of which will be stored at the Atlanta VA Medical Center in room 12C119 and on the VA server): i.Mini-Mental Status Exam, standard version ii.Barthel index iii.Lawton scale iv.Cornell Scale for Depression in Dementia v.Neuropsychiatric Inventory, brief version vi.Caregiver Strain Index vii.Zarit Caregiver Burden Scale b.A life story interview to incorporate reminiscence therapy into the design of the implementation of the Simple C Companion program c.Creation of a unique Life Story menu d.Identify individualized goals in collaboration with formal caregivers and/or family members and/or members of the individual's immediate social network 3.License Requirements. Must include: a.Include SimpleC Companion user license for one (1) year with 25 individualization units b.Require a password to access the software c.Include evidence of research and development about the user interface and redesign of the software on the portable devices d.Include evidence of use in the Atlanta area e.A demonstrated method of assisting with the following activities: i.Meaningful engagement with the resident/subject ii.Medication management iii.Improve sleep onset latency, sleep duration, and sleep quality iv.Decrease agitation v.Increase socialization vi.Increase hydration vii.Increase food consumption viii.Assist with other activities that address: 1.Behavior and mood issues 2.Challenges with Activities of Daily Living 3.Challenges with communication and participation 4.Cognitive stimulation (i.e. games appropriate for individuals who have demonstrated symptoms of cognitive impairment) 6. PERIOD OF PERFORMANCE Base Year: September 1, 2015 - August 31, 2016 Option Year 1: September 1, 2016 - August 31, 2017 7. PLACE OF PERFORMANCE Atlanta VA Medical Center's Community Living Center, Atlanta VA RR&D Center of Excellence for Visual & Neurocognitive Rehabilitation, and homes of enrolled participants in the research study. 8. Work Hours: Initial onsite training, device setup, and installation shall occur during normal business hours and is not to be performed during federal holidays as outlined below. Technical assistance shall be made available Monday-Friday from 9:00-17:00EST for the duration of the contract. G. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS The vendor/contractor shall be required to comply with all security policies/requirements. All security policies/requirements must be met cleared prior to the contractor performing work under this contract. The contractor shall identify, by name, the key management and technical personnel who will work under this task order at the time the work is being negotiated. The Contractor shall remove any employee from the performance of this contract within five (5) workdays of receiving notice from the Contracting Officer that the employee's performance is unsatisfactory. All Contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons. "The contractor shall submit a resume of qualifications to the COTR for all direct employees proposed for the project. "All Contractor employees will be approved by the COTR prior to bringing on duty. The following certifications and training are required by all contracting staff who will have direct contact with research participants from either study: "Attend all VA and project specific trainings including but not limited to: oRequired VA Talent Management System Trainings "VA Privacy and Information Security Awareness and Rules of Behavior "Privacy and HIPAA Training "V07 - Infection Control: Bloodborne Pathogens and Tuberculosis oRequired VA Collaborative Institutional Training Initiative "VA Human Subjects Protection and Good Clinical Practices "VA ORD Biosecurity Training oRequired Atlanta Research and Education Foundation Risk Assessment oRequired Human Studies Orientation "Complete all paperwork necessary to become WOC credentialed at the Atlanta VA Medical Center "Any other trainings required to become WOC credentialed at the Atlanta VA Medical Center, and required for approval by the IRB and VA R&D Committees H. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 3. Contractor shall not release medical records or other documents directly to Veterans or their designee(s). Contractor shall recognize all medical records as property of the VAMC and provide copies of all medical records to the VAMC. Veterans may request copies of their medical records by completing a written request through the VAMC Medical Records Office. 4. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. I. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to subcontractor personnel requiring the same access. 1.Position Sensitivity - The position sensitivity has been designated as moderate risk. 2.Background Investigation - The level of background investigation commensurate with the required level of access is Minimum Background Investigation - this will be conducted during the VA WOC credentialing process. 3.Contractor Responsibilities a.The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship, and are able to read, write, speak and understand the English language. b.The contractor shall submit or have their employees submit the required forms (SF 86 or SF 85P, SF 85P-S, FD 258, Contractor Fingerprint Chart, VA Form 0710, Authority for Release of Information Form, and Optional Forms 306 and 612) to the VA Office of Security and Law Enforcement within 30 days of receipt. c.The contractor, when notified of an unfavorable determination by the Government, shall withdraw the affected employee from working under the contract. d.Failure to comply with contractor personnel security requirements may result in termination of the contract for default. e. Contractor will be required to create an account in the Talent Management System (TMS) and take the Privacy & HIPAA training course #10203 before being given access and annually thereafter. 4. Government Responsibilities a.The VA Office of Security and Law Enforcement will provide the necessary forms to the contractor, or to the contractor's employees, after receiving a list of names and addresses. b.Upon receipt, the VA Office of Security and Law Enforcement will review completed forms for accuracy, and forward the forms to the office of Personnel Management (OPM) to conduct background investigations. c.The VA Office of Security and Law Enforcement will notify the CO, and contractor, of adjudication results received from OMB. 5. PROCEDURES FOR ACCESS: Access requirements to VA information systems by contractors and contractor personnel shall meet or exceed those requirements established for personnel as described in VHA directives. A written and signed request for user access by VHA management, or designee(s), constitutes management approval (sponsorship) to initiate a request for access to any sensitive automated information system. Access shall be granted to non-VA users only if the purpose for access meets criteria of the Privacy Act and VA Confidentiality Regulations and Transfer. 1.1.Contractor's personnel shall be required to sign-in before starting work under this contract and abide by the VA Computer Access Security Agreement and Confidentiality Agreement. 1.2.In the performance of official duties, the contractor's personnel may require access to printed and electronic files containing sensitive information, which must be protected under the provisions of the Privacy Act of 1974, HIPAA and other applicable Federal laws and regulations. The employee is responsible for (1) protecting that information from unauthorized release or from loss, alternation or unauthorized deletion, and (2) following applicable regulations and instructions regarding access to computerized files, release of access codes, etc. as set out in a computer access agreement which the employee signs.
