This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (1) The solicitation number VA259-15-Q-0451 is issued as a request for quotation (RFQ). (2) This solicitation is under NAICS code 339112. (3) Contract Line Item number:1 (4) Description of requirements for the items to be acquired: Line Item Product#DescriptionQty 1PS200-04071393 DEVICE, ELECTROSURGERY, 24 PEAK PLASMABLADE, WIDTH 4MM, DISPOSABLE, SPATULA TIP (4 per Box) Item Descriptions and Specifications Peak Plasma Blades Scope The essential requirement is for the contractor to provide Brand Name or Equal to the Peak Plasma Blade. If quoting an Equal product must provide a side-by-side features comparison. This will be a firm fixed-Price contract. Initial delivery of 3 boxes (12 Units) Peak PlasmaBlade, is to be delivered upon receipt of award. Orders for additional quantities up to the maximum annual quantity are to be replaced as needed by authorized Salt Lake City VA Logistics staff. This request includes the option to increase quantities of items if additional needs above the annual estimate are identified. Background The Peak PlasmaBlade is specifically utilized for the Pulsar II Generator. The PULSAR ® II Generator is the cornerstone of the PEAK ® Surgery System. It supplies unrivaled waveforms for pulsed plasma-mediated surgery and powers the PEAK PlasmaBlade, a soft tissue dissection device. The Pulsar II Generator enables surgeons to dial in the optimal amount of pulsed plasma radiofrequency energy for the PEAK PlasmaBlade device. The PULSAR II Generator is part of what makes the PEAK Surgery System simple and easy to use. It features: oAn intuitive user interface - design is based on direct feedback from surgeons and operating room staff o"Plug-and-play" functionality - PEAK PlasmaBlade disposable surgical instruments communicate back and forth with the PULSAR Generator to program recommended settings, monitor Patient Return Electrode connectivity, and make slight power adjustments based on target tissue impedance (5) Delivery: 15 Days ARO, FOB Destination, Deliver To: VA Health Care System Bldg 7 Warehouse 500 Foothill Drive Salt Lake City, UT 84148-0001 (6) Clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (7) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Product shall meet or exceed the specifications listed in the line item. Any substitution must provide detailed product descriptions and justification as to how the product deemed met or exceeded. No partial quote or shipment will be accepted. (8) Offeror shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (9) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (10) Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition with consideration to 52.219-6, Buy American Act-Supplies.
