W-9 required for consideration Required Vendor Forms Waste Removal-Dumpsters Scope of Work THIS IS A PROCUREMENT OPPORTUNITY WITH ADAMS AND ASSOCIATES AND THE GADSDEN JOB CORPS CENTER AND NOT WITH THE U.S. FEDERAL GOVERNMENT. We are currently soliciting bids for a Blanket Purchase Order (BPA) for the Gadsden Job Corps Center for the month of July with additional four month and six month options. Please see below the scope of work and service: Scope of Work: Waste Disposal Dumpster Pick-up Rate: 1. Empty (1) 8 Yard Dumpster Cafeteria (6 days/wk) based on a monthly rate. 2. Empty (1) 8 Yard Dumpster Academics (2 days/wk) based on a monthly rate. 3. Empty (1) 8 Yard Dumpster SIB Area (5 days/wk) based on a monthly rate. Please include with your bid: W-9; Vendor Questionnaire along with bid (attached) If you are a HUB Zone or disadvantaged (8a) business; please include a copy of your certification.
