- CREDIT RELEASE FORM PAST PERFORMANCE FORM SOLICITATION DOCUMENT This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 and 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number is CT2191-15 and this solicitation is issued as a Request For Proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. The North American Industry Classification System code is 313230 - Nonwoven Fabric Mills. This requirement is a total small business setaside. UNICOR, Federal Prison Industries, Inc., intends to enter into a one(1) year firm-fixed-price, requirements type contract for the following commercial item(s): Nonwoven wool cloth for disaster blankets **************************************************************** ALL ITEMS ARE REQUIRED TO BE BERRY AMENDMENT COMPLIANT THIS ITEM IS 100% USA DOMESTIC ONLY, MANUFACTURED ENTIRELY IN THE UNITED STATES OF AMERICA. THIS IS TO INCLUDE THE RAW MATERIALS. THE NON-MANUFACTURER RULE APPLIES - A RECIPIENT OF A CONTRACT SET-ASIDE FOR SMALL BUSINESS, IF OTHER THAN THE ACTUAL MANUFACTURER, MUST PROVIDE THE PRODUCTS OF A SMALL BUSINESS. ****************************************************************
