VA243-15-N-1091_1 ****Request for information only**** EBOLA PERSONAL PROTECTIVE EQUIPMENT VISN 3 (NEW YORK/NEW JERSEY) HEALTHCARE SYSTEM This request for information (RFI) is posted for market research purposes ONLY. It is not a solicitation. The Department of Veterans (VA), Network Contracting Office 3 (NCO3) seeks to establish a VISN 3 local Blanket Purchase Agreement (BPA), to acquire supplies for Personal Protective Equipment (PPE) meeting CDC Guidelines to treat Ebola patients and maintain any current changes as needed. There is a Patients Prime Vendor Program (PVP) currently in place through Cardinal Healthcare, therefore this request should be a 100% utilization of the Prime Vendor Program. The product shall meet the features and specifications referenced below: Requirements Please submit the names of the brands that you are able to provide with your RFI response. 1)Two Options are included for respiratory protection: N95 Respirator or Powered Air Purifying Respirator (PAPR) ( All VISN 3) Facilities If a NIOSH-certified fit-tested disposable respirator (e.g., N95 respirator) or NIOSH-certified PAPR are used in facility protocols, ensure compliance with all elements of the OSHA Respiratory Protection Standards including, fit testing, medical evaluation and training of the worker. 2)Single-use Level IV (disposable), fluid-resistant or impermeable apron that covers the torso to the level of the mid-calf should be used if patients with EVD have vomiting or diarrhea. ( All VISN 3 Facilities ) 3)Tyvek Protective Suit ( All VISN 3 Facilities ) FOB: Destination All of these items must be in compliance with CDC Guidelines. Time of delivery: These items must be available during an emergency response identified by the agency, VISN or individual facility. All interested parties should provide the following information via e-mail only to the Contracting Officer by the stated deadline: 1.The company's ability to supply the product requested 2.The company's NAICS Code 3.Socio-economic category under which the vendor is categorized 4.Company Point of Contact information Response date (Deadline): Friday July 3, 2015 at 4:00 (EST) For planning and scheduling purposes, to insure being included in our market research for this product, responses must be received no later than close of business on Friday July 3, 2015.
