Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-15-R-0457 is issued as a request for proposals (RFP). The North American Industry Classification System (NAICS) Code 333318 Other Commercial and Service Industry Machinery Manufacturing and a Business Size Standard of 1,000 employees is applicable. This is a small business set-aside procurement. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83. No telephone requests will be accepted. A walk through is scheduled for Thursday, July 16, 2015 at 4pm, Building 1, Room B108. It is strongly suggested that all interested parties attend this walk through so the full extent of work can be verified prior to providing a proposal for this procurement. Proposals are requested for the following: Scope of Work Contractor shall furnish all labor and materials and perform the work for a complete installation of one monochloramine system to proportionally dose 2-4 ppm of monochloramine into our domestic hot water plumbing system. The contractor is responsible for following all applicable local, state, and Federal building codes. The equipment shall meet the following specifications: "Flow through reactor made of C-PVC "Electronic Controller for the operation of the system "Volumetric dosing pumps for the reagents "Reagent lines made of PVDF plastic with foot valves "Flow sensor for the reagent lines "Flow sensor for the pre-dilution loop "Vent system "Pre-dilution circulating pump and circuit made in CPVC "Remote-control device with an integrated web server to operate the system from the Internet (requires the Internet access with a static IP address) " On-line analyzer for ORP, redox potential, "One(1) Magnetic flow transmitter "One(1) Containment skid for Reagent tanks "One(1) Monochloramine Colorimeter w/reagents "Maximum Capacity: 060 g/h Installation shall include: "All work necessary to make equipment a working system per building codes "Furnish & Install required PVF for new system and circulation pumps "Removal of existing system "Water Meter Connections "Electrical oDisconnect existing system oConnect new system oConnect new pumps with disconnects "Temperature Controls oProgramming to provide flow back to Monochloramine system "Insulation of new piping Operational Supplies: "One month supply of chemicals. Training: "Comprehensive operator training 4 hour session offered twice (on different days to be determined after award) at start up and then as required by the facility on a separate purchase request. Please quote each training session as a separate line item in your proposal. Warranty information must be supplied once award is made and should include: "Annual Preventative Maintenance and Maintenance Kit "24/7 Online Monitoring "On-Line Log Book: "Monthly Service Visits for length of Warranty to include the following Items oDomestic Water Analysis oTreatment Log Review oCalibration of Controls oChemical Inventory Record oAdjust Chemical Application Levels "Quarterly for length of warranty provide the following items: oDosing Pump Calibrations "Annually for length of warranty provide the following items: oReplacement of Dosing pump liquid ends oOperator Training oFormal Program Review "24 Month Warranty If these warranty items are not covered in the product/service commercial warranty please provide as part of your proposal the cost of having these items added as an extended warranty for a period of 12 months with the service period beginning the first full day after install is completed. System to be installed at the Saginaw VAMC, 1500 Weiss Street, Saginaw, MI 48602. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) to VA Financial Services Center, P.O. Box 149971, Austin, Texas 78714-8971 through the OB10 electronic invoicing system. Full text of the following clauses and/or provisions can be viewed through Internet access at http://farsite.hill.af.mil. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Offerors shall submit with their offer, a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items or confirm this has already been completed in their SAM registration. The clause at 52.212.4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.232-34. All questions must be submitted in writing to Calynda.baines3@va.gov. Offerors shall submit their proposals via the same email address. Proposals are due no later than 4:00pm. EST, 24 July 2015. NO LATE PROPOSALS WILL BE ACCEPTED OR CONSIDERED. The successful offeror must be registered with SAM to be awarded a contract.
