1.Can you provide the actual volume for the Michael E. DeBakey VA Medical Center for the past three years? If that information is not available, could you provide an average? Five cases per month average, varies 2 - 10. 2.What is the preferred day(s) of the week/month and are treatments to be schedule once per month? Preferred day is the last Tuesday of the month - typically. 3.How is the proposal to be accepted: e-mail/fax/mailed? Email or fax is acceptable. 4.Does the facility require the lithotripter along with an imaging system as well or do they already have one? Both lithotripter and imaging system is required.
