Description

This solicitation VA246-15-Q-1028 is issued as a request for quote (RFQ). Award will be made using simplified acquisition procedures in accordance with FAR part 13. No telephone requests will be accepted. Only completed SF1449 Solicitations received directly from the requester are acceptable. It is the Offeror's responsibility to ensure that the quote is received. The anticipated award date is August 3, 2015. The associated North American Industry Classification System (NAICS) code for this procurement is 339112. This action is set-aside to Service Disabled Veteran Owned Small Businesses (SDVOSB). If offeror has a FSS/GSA contract, please include it in your offer. The Department of Veterans Affairs, Salisbury, NC is seeking to purchase a Brand Name or Equal to the Olympus Holmium Laser System for the under construction health care facility in Kernersville, NC. Delivery of equipment is to be scheduled for January 6, 2016. "Equal" equipment must meet the following salient specifications: "Laser energy for eliminating stones with maximum control and minimal tissue damage on all types of stones, regardless of composition. "Up to 30 watts of laser energy to effectively eliminate stones. Variable pulse energy and frequency is optimal for stone fragmentation, tissue ablation, incision, or coagulation. "Small spot size to increase efficacy and output of fiber. Laser is easily seen for aiming in the laser fiber's surgical field. Fiber recognition system provides the user immediate information regarding the connected fiber. "Wireless foot pedal for control of the laser. "Space saving, floor base on wheels to move easily from one room to another. "Plug to fit standard outlet, without need for special water or other supplies, and no warm-up period (instant use). Interested and capable respondents are requested to provide the information below. All responses to this notice shall include: company name, address, point of contact, phone number, DUNS number (www.dnb.com ), list of available products, the geographical location the respondent is able to service, and a list of the respondent's establishment's certifications from private, State and/or local agencies documenting the safety and sanitary conditions and procedures. Responses to this notice should be emailed to Lori Miller, Contracting Officer at e-mail address: lori.miller@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than 2:00PM EDT, July 30, 2015. All contractors interested in doing business with the Government must be registered in the System for Award Management (SAM) database. The website for registration is www.sam.gov. Service-Disabled Veteran-Owned Small Businesses should also indicate whether the company is verified by the VA in the Vendor Information Pages (VIP) Database at www.vetbiz.gov. If an "Equal" product is being proposed, a full product description, make and model shall be submitted with sufficient technical detail to allow the Government to determine the item to be at least equal to the brand name items listed above. Offers without this information cannot be evaluated and the quotation will be determined to be non-responsive. A detailed description of agreement terms and conditions, any maintenance services, and customer service shall be included with the quote. Submitted quotations shall be done using the SF1449 VA246-15-Q-1028 and include company name, and point of contact, address, phone number, fax number, tax identification number, DUNS number, and business size.
