Description
Statement of Work Adaptive Surf Instruction for Surfing Venue 1.Scope: Monday, September 14 - 18, 2015 The contractor shall provide Surfing Instruction for the VA National Veterans Summer Sports Clinic (NVSSC), VA San Diego Healthcare System, San Diego, CA in accordance with this contract. Qualifications of Surf School are as follows: "Owner/Director must have a minimum of 25 years proven experience in surf instruction with a permitted/insured/bonded/outfitter in the U.S. "Proven record of providing adaptive surf instruction to 130 surfing participants during one evolution of instruction for duration of five consecutive days. "Organization must have experience with a diverse population of persons with disabilities most importantly wounded, ill or injured service members, and Veterans. Example - "disabilities ranging from Post-Traumatic Stress Disabilities - Spinal Cord Injuries, Burns - Blindness, and Amputations - Traumatic Brain Injuries." "Organization must have held surfing clinics providing adaptive surf instruction to 110 surfing participants during one evolution of instruction for duration of five consecutive days within the past 2 years "Organization must provide documentation of providing adaptive surf instruction to 110 surfing participants "Local knowledge/community approvals "School must be permitted to operate a surf camp within the City of San Diego Surf Camp RFP requirements "School approved by the La Jolla Shores Association and have a business license in La Jolla, CA current as of January 1, 2015 "Must carry $3,000,000.00 total (aggregate) insurance "Must carry Worker's Comp Insurance for all instructors and staff: no independent contractors or subs "Must employ at least 40% female employees, including support staff, instructors, and managers "Instructors must all be certified in CPR, First Aid, and Lifesaving by the American Red Cross "Certified Level 1 Surf Instructor by the ISA "Must be able to pass fingerprint/background check with LiveScan via the Department of Justice September 14 - 18, 2015 Surfing Instruction shall be provided at one venue location; La Jolla Shores - Kellogg Park. Surfing Instruction shall be provided for approximately 40 people per day. 1.Acronyms: NVSSC - National Veterans Summer Sports Clinic; VASDHS - VA San Diego Healthcare System; COTR - Contracting Officer's Technical Representative; 2.Privacy Act: The contractor shall comply with the provisions of the Privacy Act of 1974, HIPAA and all applicable VA laws and regulations. Additional information to identify the patient and type of document will be provided by the VA SDHCS as needed. The Contractor shall not maintain, in any form, any sensitive or patient identified information. The Contractor shall comply with the Privacy Act and HIPAA as it pertains to general patient information, HIV, Drug and Alcohol Abuse, and Sickle Cell Anemia. The Contractor shall have plant security and ADP security necessary to safeguard VA records as needed. 3.SPECIAL CONTRACTOR RESPONSIBILITES: 1.The contractor shall designate a primary and alternate point of contact by name and phone number who will function as liaison with the COR on day-to-day operations, and will be available from 6:00am PST to 4:30pm PST Saturday, September 12 through Saturday, September 19, 2015. 2.The Contractor shall designate a company officer authorized to resolve ongoing problems that have not been resolved at the operations level. 3.Closeout Deadlines: VA NVSSC is required to meet weekly closeout deadlines. The contractor shall ensure that VA does not fail to meet this deadline due to contractor failure to meet the contract.
