- THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTES; THE VA IS NOT SEEKING PRICING OR OFFERS. The Department of Veterans Affairs, Network Contracting Office 20, in support of VA Puget Sound Health Care System, is performing market research to compile data regarding: (1) The availability and capability of qualified small business concerns; (2) Identify if there are sufficient number of qualified Service-Disabled Veteran-Owned Small Business, Veteran Owned Small Business, HUB Zone 8(a), Women-Owned Small Business, or Small Disadvantaged Business, Small Business concerns relative to the subject North American Industry Classification System (NAICS) code and large businesses. Responses to the information requested will assist the Government in determining the appropriate procurement method, including any set-aside decision. Respondents should clearly describe their capabilities to include management experience, equipment and personnel or the ability to obtain them. Furthermore, interested sources must also meet all requirements of Federal, State, and Local Municipal codes regarding operations of this type service. The NAICS code assigned is: 611620 (Sports and Recreation Instruction) and the small business size standard is $7.5M. Not responding to this Sources Sought does not preclude participation in any future or potential solicitation, or Request for Quotation. It is the intent of Network Contracting Office 20 to use the information gathered for market research purposes only. If a formal solicitation is released, it will be posted via the Federal Business Opportunity (http://www.fbo.gov) DESCRIPTION OF THE REQUIREMENT: The scope of this contract is to provide all necessary labor and materials for Veteran-only free swim and structured aerobic swim class. The contract will reserve the pool facilities for one-hour sessions three times a week. The sessions will be on Monday, Wednesday, and Friday from 2 pm - 3 pm PST. The pool shall be available for Veteran-only swim during the sessions. Veterans may attend any two of the three one-hour sessions per week. The Contractors pool facility shall be large enough to house 105 Veterans at each session. The Contractors pool facility shall be located in the North Bend, Oregon area. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1. Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size i.e. SDVOSB, VOSB, 8A, etc., and DUNS Number. Proof of System for Award Management (SAM) Registration and Vet Biz registry (if applicable) is highly desirable. Identify all Joint Ventures and other Teaming arrangements. 2. A brief capability statement demonstrating past performance with relevant contract services in Prime Contractor role. 3. Disclaimer: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Responses must be submitted via e-mail to: Ross.Byrne@va.gov Note: Do not include proprietary, classified, confidential, or sensitive information in responses. **Draft Statement of Work STATEMENT OF WORK 1.0 TITLE: AEROBIC & FREE SWIM PROGRAM 2.0 BACKGROUND: The Roseburg VA Healthcare System and the Health Promotion Disease Prevention (HPDP) have a mission to improve Veteran health and wellbeing through the promotion of programs to support a healthy lifestyle and lifestyle change. HPDP implements programs that increase physical activity and promote healthy eating. Through partnerships in the community the Veterans are granted greater access to such opportunities. 3.0 SCOPE: The scope of this contract is to provide all necessary labor and materials for Veteran-only free swim and structured aerobic swim class. The contract will reserve the pool facilities for one-hour sessions three times a week. The sessions will be on Monday, Wednesday, and Friday from 2 pm - 3 pm PST. The pool shall be available for Veteran-only swim during the sessions. Veterans may attend any two of the three one-hour sessions per week. 4.0REQUIREMENTS: The Contractor is responsible to provide the following: a. Supplying all equipment, materials, and instructors to accomplish task b. Supplying all required safety equipment and lifeguard personnel c. Provide leading and coaching to the Veterans in the class. The Contractor shall track attendance and shall provide information on the number of Veterans who use the pool on a weekly basis and number of times each Veteran uses the pool. This information shall be provided in an aggregated and anonymous format. d. The Contractors facility shall be capable of supporting up to 105 Veterans, per each session e. The Contractors pool shall be divided to accommodate lap swimming/open exercise and an instructed aerobic exercise class. f. The Contractors pool shall be in an indoor temperature controlled facility g. The Contractor shall provide a qualified aerobic swim instructor h. The Contractors facility shall be located in the North Bend, OR area. 5.0 VA RESPONSIBILITIES: A VA point-of-contact shall be appointed for all program-related issues 6.0PERIOD OF PERFORMANCE: See B.2 Pricing Schedule 7.0 SECURITY REQUIREMENT: The Contractor employees shall not have access to VA sensitive or computer information and will not require routine or intermittent access to VA Facilities. No background investigation is required. 8.0DELIVERY SCHEDULE: The Contractor shall email the Veterans Affairs Point of Contact each Friday with the number of Veterans who attended sessions that week.
