Description
Contractor shall provide semi-annual waste water testing in accordance with the requirements of the City of Waco Significant Industrial User Wastewater Discharge Permit # COW-9202, for the Central Texas Veterans Health Care System (CTVHCS), 4800 Memorial Drive, Waco, Texas 76711. A copy of the permit is attached to this Statement of Work. Contractor shall also provide waste water testing in accordance with the requirements of the City of Temple Significant Industrial Permit Number DS-9202, with modification to include annual testing vs. semi-annual. As of January 9, 2013, the Temple Olin E. Teague Veterans Center has been removed from the City of Temple's List of Significant Industrial Users. Wastewater testing shall continue as prescribed in the permit, but the frequency of testing will be adjusted to a yearly requirement. A copy of the permit is attached to this Statement of Work. 1.0 REQUIREMENTS: 1.1.General Obligations: Contractor shall furnish all necessary transportation, labor, supervision, supplies, and equipment to pick-up and analyze waste water samples from the Temple or Waco facility. 1.2.Packing and Shipping: Contractor shall ensure all packing and shipping of collected wastewater samples are in accordance with federal Department of Transportation (DOT) regulations, including the proper fitting of lids for use. All transportation of wastewater samples shall be sent to an EPA or TCEQ approved analytical facility. 1.3.Spill Response and Control: Contractor will have an adequate provision of spill supplies and equipment on site to control inadvertent spills caused during the collection, packaging and transport of waste samples. Contractor shall clean up any spilled material during collection, packaging and transfer of waste water back to background levels prior to the spill or to the satisfaction of the CTVHCS. 1.4Reporting of Unknown Hazardous Chemicals: Should the Contractor ever detect unknown hazardous chemicals, they shall immediately report the finding to the Contracting Officer Technical Representative (COTR) and work with the facility regarding security, holding or return of any packing and shipping container found to contain an unknown hazardous chemical. 1.5Presample Notification: Contractor shall notify COTR at least one week in advance of sample collection so VA Police can be notified that they will be on site. 1.6 Monitoring Requirements: Monitoring requirements for the Waco VA are addressed in the attached Permit #COW-028. Wastewater samples will be collected and sampled for the specified type of pollutants and at the specified sampling frequencies. Monitoring requirements for the Temple VA are addressed in attached permit DS-9202. Wastewater samples will be collected and sampled for the specified type of pollutants, but at a modified sampling frequency of once per year. While exact dates for sampling is at the discretion of CTVAHCS, contractor proposal dates will receive the highest consideration. 1.7 Analytical Results- Contractor shall provide sampling results within 24 - hours of analysis completion and quality assurance verification. 2.0 REGULATORY COMPLIANCE: 2.1. Documentation- The Contractor shall provide CTVHCS all documentation as prescribed by and in accordance with Waco Permit #COW-028, or Temple Permit #DS-9202. 2.2 Chain of Custody and/or Bill of Lading: A chain-of-custody and/or bill-of -lading shall be completed prior to departure when waste water samples are obtained. Contractor shall sign, date, and furnish a copy of the chain-of-custody and/or bill-of -lading to the COTR with the analytical results. 3.0 PERMITS/LICENSURE: 3.1. Changes: The Contractor shall notify the COTR of any change or alteration to their certification, permits and licenses for facilities, (including but not limited to water analysis), or for any change in status which may affect performance. 4.0 FEDERAL HOLIDAYS: 4.1 The following national holidays are observed: New Year's Day 1 January Martin Luther King's Birthday 3rd Monday in January President's Day 3rd Monday in February Memorial Day Last Monday in May Independence Day 4 July Labor Day 1st Monday in September Columbus Day 2nd Monday in October Veterans Day 11 November Thanksgiving Day 4th Thursday in November Christmas Day 25 December 4.2. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a oliday falls on a Saturday, the preceding Friday shall be observed as a legal holiday.
