Description

This is a Notice of Intent. The North American Industry Classification System (NAICS) identifier for this service is 541519 with a size standard of 38.5M. This does not constitute a solicitation. The information gathered from this Notice of Intent is for planning purposes only. The Government is not obligated, nor shall it pay for, any information received from potential sources as a result of this notice. The purpose of this notice is to advise of the intent to sole source the following item to VISN 2 VA Medical Centers: Form Web 2: This procurement is for the renewal of the software license and hosting fee for Formulary Productions, LLC Electronic Formulary and Pharmacy Applications product. The proposed contract period is anticipated to be for a base period of October 1, 2015 - September 30, 2016 plus 4 option years. The proposed contract will be a firm fixed price contract. Contract performance is subject to availability of funds. Responses to this notice must be submitted by Wednesday, September 9, 2015 @ noon. Offerors who have the ability to provide this licensed product shall submit a response by email or fax. All questions in reference to this sources sought notice shall be submitted in writing to include a point of contact, telephone number and an email address.
