- Please read the attached file for more details -- VIPER SF 1449 Solicitation (FINAL) Combined Synopsis/Solicitation FA3047-15-T-0019 CERTIFIED ATHLETIC TRAINING SERVICES TO SUPPORT VERSATILE INJURY PREVENTION AND EMBEDDED RECONDITIONING (VIPER) MISSION 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. The solicitation number is FA3047-15-T-0019 and is issued as a Request for Quotation (RFQ). 3. The solicitation document and incorporated provisions are those in effect though Federal Acquisition Circular 2005-83 and DFARS change notice 20150626. 4. This is a notice for 100% Total Small Business Set-Aside. The NAICS code is 621340 and the size standard is $7.5M. 5. The contract line items numbers and items, quantities and units of measure pertaining to this requirement are identified on the solicitation. The solicitation with statement of work is available for download via www.fbo.gov. Quotes shall be on an all or none basis. 6. The contractor personnel (CP) shall provide Certified Athletic Training Services supporting the VIPER mission. The CP shall work within the Government provided facility or in field conditions in varied weather conditions (on or off military installations throughout the Joint Base San Antonio (JBSA), TX area). Transportation to field condition locations will be provided by Government. CP shall monitor Trainees in activities such as, but not limited to, land navigation, patrolling, heat mitigation, and small unit tactics. 7. Period of Performance will be a Base + 4 Option Years: a. BASE YEAR: 30 September 2015 - 29 September 2016 b. 1ST OPTION YEAR: 30 September 2016 - 29 September 2017 c. 2ND OPTION YEAR: 30 September 2017 - 29 September 2018 d. 3RD OPTION YEAR: 30 September 2018 - 29 September 2019 e. 4TH OPTION YEAR: 30 September 2019 - 29 September 2020 8. Submission of quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items and Addendum. Failure to comply with the terms and conditions of the solicitation may result in the Quoter being ineligible for award. 9. Quoters must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes. 10. To be eligible to receive an award resulting from this solicitation, registration with the System for Award Management (SAM) must be current, no exceptions. The System for Award Management is the DOD Central Contractor Registration (CCR) database. To register, please visit https://www.sam.gov/portal/public/SAM/. 11. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition to include Addendum 52.212-4 details of which are included in the solicitation. 12. The clause at 52.212-5 (DEVIATION), Contract Terms and Conditions Required to Implement Statutes Or Executive Orders - Commercial Items, applies to this acquisition details of which are included in the solicitation. 13. Please send all questions no later than 4:30pm CST on 8 September 2015, any questions sent after that will be considered late and does not guarantee a response.
