Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number VA260-14-Q-1132 is being issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-83, dated July 02, 2015. The North American Industry Classification system code (NAICS) 541512 with a small business size standard of $27.5M dollars. This acquisition is set-aside for small businesses. All firms responding must be registered with the System for Award Management (SAM). Offerors quotes shall propose a firm fixed price for the supplies/services listed below, subject to the terms and conditions of this entire synopsis/solicitation. The location for performance is: Department of Veterans Affairs VA Portland Health Care System Vancouver Warehouse 1601 E. Fourth Plain Blvd Vancouver, WA 98661 Quotes must be received by September 15, 2015 4:00PM EST. Quotes must be in writing, and submitted by email. Facsimiles will not be accepted. Description: The Network Contracting Office 20 is soliciting competitive offers for a Brand name or equal to Zeiss Forum View System in accordance with the attached solicitation document. The government intends to award a single firm-fixed price contract, without discussions, to the offeror who represents the best value to the government, based on the evaluation factors outlined under FAR 52.212-2 in the attached solicitation document. Quotes are requested to be FOB Destination, and inclusive of shipping charges. Quotes will only be accepted for new, unused items. Quotes for used or refurbished items will not be considered. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. (9) Any other information required by this solicitation. By submitting an offer, the offeror agrees to hold the prices in its offer firm for at least 30 calendar days from the date specified for the receipt of offers, unless another time period is specified in an addendum to the solicitation. No phone calls will be accepted. Answers to questions will be posted as an amendment to this solicitation as necessary. Individual responses are not promised, and should not be expected. Clauses and provisions from the Federal Acquisition Regulation (FAR) and supplements thereto are incorporated in this solicitation by reference and in full text. Those incorporated by reference have the same force and effect as if they were given in full text.
