PROJECT: Orlando VA Medical Center is seeking a GE certified technician to repair one Optima XR220amx Digital Mobile X-Ray System at our Community Living Center located at Lake Nona VA Medical Hospital, 13800 Veterans Way, Orlando, FL 32827. BACKGROUND: The Orlando VA Medical Center (Lake Nona) is conducting market research for information from all interested vendors regarding repair, and calibration of one Optima XR220amx X-Ray machine. PART NEEDED FOR REPAIR: One Bipolar HV Tank, GE part number 5351748 NAICS Code: 334510 REQUESTED INFORMATION: You're encouraged to provide any additional information that may be useful when answering the below questions. 1) Name and address of your business: 2) Provide your DUNS Number: 3) Does your business offer the services listed above (see "Background")? 4) Are you a certified elevator technician / reseller? 4) Is your business a United States business?
