- Waiver of Claims W9 Vendor Questionnaire AT2015-115 FIRE ALARM INSPECTIONS AND REPAIRS Adams and Associates, Inc., contracting for the U.S. Department of Labor for the operation of the Atterbury Job Corps Center - a 410 student residential training facility located at 3129 E. Edinburgh Street, Edinburgh, is currently soliciting written bids for Fire alarm inspection-repairs. A mandatory walk through will be conducted on Thursday, 9/17 at 9:00 am eastern time. Bidder must be present for walk through for bid to be considered. All quotes should be submitted in writing to the Purchasing Department by Monday, September 28, 2015, 3 pm eastern time. Quotes received after the deadline and/or without all requested documentation will not be considered. Vendor is to return written bid on vendor letterhead, vendor paperwork, and all licenses for doing business in the State of Indiana, all employee licenses required for performing work in the state of Indiana, Certificate of Liability insurance and list of references if no past experience with Center. Vendor must include the Scope of Work Identification number on submitted bid. The period of performance is effective 10/1/2015-9/30/2016. Submitted bid will show at minimum breakdown as follows: • Breakdown of Annual Services • Fire Extinguisher Service as Mandated by State Code – Annual • Kitchen Fire Suppression Service – Perform Semi-Annual • Fire Sprinkler System Service – Annual • Fire Alarm System Service • Labor Cost per Hour for Service Calls as needed to maintain system through out the year in addition to annual services • Printed name and Signature of authorized person submitting bid If your quote deviates from specifications in any way, such exceptions must be noted on bid sheet. If unable to quote please indicate ‘No Bid’ and return bid package to us. Adams and Associates, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly. Bidders are placed on notice that this is a competitive bid process and cost is an important factor. Overall low bid does not constitute bid award. Atterbury Job Corps Center is a federally funded facility and entitled to GSA pricing. Payment terms are Net 30 days from date of invoice. Vendor is required to show number of hours billed and hourly rate on invoice, list of materials used to complete service and provide certified payroll records with invoice. Payments for award are to be billed in (12) equal payments. All wages are to be paid per Service Contract Act. Edinburgh Street P O Box 187 Edinburgh, Indiana 46124 SCOPE OF WORK ID #: AT2015-115 FIRE ALARM INSPECTION-REPAIRS Vendor to provide all required Fire alarm inspection-repairs as deemed necessary by Atterbury Job Corps Center. With services as follows: PAYMENTS FOR AWARD TO BE BILLED IN TWELVE (12) EQUAL PAYMENTS WITH BILLING AND PAID NET 30 DAYS. VENDOR TO SUPPLY DETAILED REPORT WITHIN 10 DAYS AFTER EACH INSPECTION. Per Item Cost for Period Breakdown of Services Annual: Inspection of Fire Extinguishers (Approximately 250) Inspection of Fire Alarm Detection Systems (Test and Inspect) Inspection of Kitchen Fire Suppression Systems (Test and Inspect) Inspection of Fire Sprinkler System (Test and Inspect) Fire Extinguisher Service as Mandated by State Code - Annual: Visually inspect extinguishers Check test and maintenance dates Weigh the extinguisher Visually inspect pressure gauge Remove safety pull pin Install new tamper seal Remove hose and check continuity Inspect valve assembly Hang extinguisher Clean extinguisher Attach new certification tag Fill out fire and Safety Report Kitchen Fire Suppression Service – Perform semi-annual: Check cylinder condition Check hydrostatic and regulator test dates Check all piping and conduit for rigidity Check and verify nozzles are free of blockage and properly aimed Kitchen Fire Suppression Service (continued) Dry test system using a test link Replace all fusible links Dry test system with manual pull station Check operation of gas/electric shut down Weigh or replace firing cartridges as required Check all nozzles for proper type of cap/covers Check chemical type and quantity Verify filter type in hood Reconnect system and remove locking tools Complete restaurant inspection report and attach inspection tag Check for proper type of portable extinguisher in kitchen area (K Class) Fire Sprinkler System Service – Annual: Perform a main drain test on all risers noting static and residual water pressure Inspect all fire department connections Dry Pip Valves – Inspect for proper air and water pressure and priming water level Verify that monitoring station receives signal from water flow supervisory Perform an annual partial trip test and drain all low points on dry system Verify monitoring station receives signal from water flow supervisory Perform a walk through visual inspection of sprinkler heads from floor level Annual inspection on DC backflow prevention devices Semi annual inspection on RPZ backflow prevent devices Fire Alarm System Service: Check fire alarm panel to ensure that the alarm initiating devices are functioning properly Verify the alarm indicating devices are functioning properly Clean detectors with compressed air Functionally test detectors with test smoke Perform sensitivity test on smoke detectors on a bi-annual basis Pull every pull station and check their accessibility If the system is monitored, verify the signal reaches monitoring station The above listed unit pricing includes necessary labor, materials, equipment, transportation, and supervision required to accomplish the Scope of Work. Any additional charges for services or product that are not included in the above items shall be noted in bid proposal. A schedule will be provided (once the project is awarded) to the winning bidder as to approximate month and day each service is to be performed. The winning bidder will be required to work around our schedule and meet our deadlines for completion of work. The work hours will be determined for each individual section. Vendor is to include hourly wage for service calls as needed to maintain system. Emergency calls will be responded to within 4 hours or sooner as determined by Purchasing Agent. Vendor shall hold all licenses necessary to do business in the state of Indiana and provide copies of all licenses with bid package. Vendor is to provide invoice that includes BPA number and show times for each pickup. The vendor shall obtain and maintain insurance coverage as follows: • Workers Compensation • Comprehensive General Liability with Atterbury Job Corps Center listed as additional insured. ($1,000,000 per occurrence). • Comprehensive Automobile Liability ($1,000,000/combined single limit) • Fire Legal Liability ($500,000 per occurrence) • The proposed shall cover all expenses not covered by said insurance including all deductibles.
