- Amendment 000001 dissolves Small Business Set-aside requirements for a full and open, non-restrictive competition, changes the period of performance, provides Price Schedule format requirement, and extends the competition. This Combined Synopsis/Solicitation Notice is to announce the U.S. Department of the Interior (DOI), U.S. Geological Survey (USGS) has a requirement to establish a Blanket Purchase Agreement for DOSIMETRY SERVICES in accordance with the FedConnect posted Solicitation G15PS00708. A copy of this combined synopsis/solicitation may be accessed at FedConnect, www.fedconnect.net. Solicitation G15PS00708 is being issued as a Request For Offers and requires both price/cost and non-price documentation for determination of the Best Value to the Government based on Technical:Price Trade-off. Attachment 1, Current Dosimetry Program Requirements, is provide for information purposes ONLY. It is not a guarantee by the Government of any future work requirements. The policies contained in FAR Part 13 Simplified Acquisition, in conjunction with the policies and procedures for solicitation, evaluation and award prescribed in Part 15, as appropriate, shall be used for this acquisition. This is a 100% Small Business Set-aside. The associated NAICS code is 541380, which has a small business size standard is $15.0M. The Non-price/cost Factors requiring offeror provided documentaton are (1) Technical Capability ¿ offerors shall be established in the required services business. Offerors shall provide a narrative, not to exceed two pages, describing their capabilities to perform the services described in the Work Statement; and (2) Past Performance ¿ offers shall submit not less than two references for services performed during the past three years. All requirements for preparation, submission, and receipt of offers contained in the solicitation shall be adhered to.
