Description

1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.603, as supplemented with additional information included in this notice and FAR 13.5, Test Program for Certain Commercial Items. This announcement constitutes the only solicitation; offers are being requested and no other solicitation will be issued. All responsible offerors may submit a quotation that is timely received shall be considered. 2. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. The provision at 52.212-1, Instructions to Offers - Commercial, applies to this acquisition. The provision of 52.212-2, Evaluation - Commercial Items, addenda to provision apply. Offers shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with offer. Clause 52.212-4, Contract Terms and Conditions-Commercial Items, apply to this acquisition. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items apply to this acquisition. Additional contract terms and conditions listed in attachments. 3. This a RFQ for the purchase of services to home oxygen services at multiple locations throughout the Orlando Veterans Affairs Medical Center's area of responsibilities. This requirement is solicited as a Small Business set-aside. North American Industry Classification System (NAICS) code NAICS 621610 Size Standard $15M. 4. The required performance start date for this RFQ is: November 13, 2015 5. This RFQ is not an authorization to begin performance and in no way obligates the Government for any costs incurred by the contractor in regards to this procurement. Offerors shall honor quote no less than 60 days after quote award. The Government reserves the right to cancel, this solicitation, either before or after the stated closing date for receipt of offers. In the event of a RFQ cancellation, the Government has no obligation to reimburse any potential Offeror for any costs incurred. 6. Submit offers electronically in accordance with the RFQ submission instructions as outlined in Attachment A, on or before, offer due date written in block #8 of the solicitation (Standard Form 1449). Time is based on local time (Eastern Standard Time). 7. Close Date for questions: Sep 17, 2015/11A.M. (EST) (submit questions via e-mail rosa.rivera3@va.gov). Phone calls not accepted.
