- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FAR SUBPART 12.6-STREAMLINED PROCEDURES FOR EVALUATION AND SOLICITATION FOR COMMERCIAL ITEMS-AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE, THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. OFFERS ARE BEING REQUESTED, AND A WRITTEN SOLICITATION DOCUMENT WILL NOT BE ISSUED. This is a solicitation W91YU0-15-T-0362 of Purchase Request 0010740029 issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-83. The associated North American Industrial Classification System (NAICS) code for this procurement is 923120; Size Standard for small business not established, listed for Public Administration of Federal, State, and local Government agencies. RFQ is being issued as UNRESTRICTED. These services are for WEED Army Community Hospital, Fort Irwin CA. 92310 for a Base Year from 01 October 2015 through 30 September 2016 and (2) One-Year Option Periods. Selection will be made on the following: All responsible Contractors shall provide an offer for that provides (at a minimum) the following screening tests for Newborn Screening Services: See Full Listing; Appendix (1) NBS- Newborn Screening Program, an attachment at the end of this solicitation. Endocrine Disorders: Congenital Hypothyroidism & Congenital Adrenal Hyperplasia Enzyme Deficiency Disorders: Galactosemia Biotinidase Deficiency Hemoglobinopathies: Sickle Cell: HbS/S, HbS/C, HbS/D, HbS/E Hb Bart's, HB H, and other abnormal Hemoglobin Disorders. Cystic Fibrosis: Metabolic Disorders to include; aminoacidopathies, fatty acid oxidation disorders, mitochondrial disorders, urea cycle disorders and organic acidemas, and all other abnormal disorders. Severe Combined Immunodeficiency (SCID) Disorders. Must use (1) one universal specimen card for both initial and follow up testing. All responsible Contractors shall provide an offer for (2) two screens for each newborn. MS/MS (Tandem Mass Spec) analysis shall be performed on both the first and second screening samples. All responsible Contractors shall provide consultation and follow up evaluation of abnormal screening results by specialist in metabolic genetics, pediatric endocrinology and hematology. These specialists shall be available by phone and respond within (2) two hours for direct patient evaluation, if required. All responsible Contractors shall provide first blood samples from newborns. These samples must be sent by Federal Express (FEDEX), or equivalent courier service. All abnormal results of an urgent nature shall be telephoned and results faxed to the WEED Army Community Hospital Metabolic Screening Program Manager or other appropriate staff, as directed.
