- See attached RFQ: 15-T-1358 - XR287 REQUEST THE FOLLOWING INFORMATION FOR REQUEST FOR QUOTES (RFQ): This solicitation is restricted (set a-side) to Small Business. Requirement to be awarded on a Brand Name or Equal basis. Technical evaluation of vendor's quote will consist of technical capability to provide and perform product specifications as stated in Section B & price. This RFQ is a requirement for a OPTIMA SR220amx Upgrade and training. Please see section B for all part numbers and sizes. These items are manufactured by GE Healthcare and will be used by the Radiology Dept. at Naval Medical Center-Portsmouth, VA. Quotes should reflect pricing on all items for consideration. Please provide government & destination pricing. Also, state warranty information for each CLIN number (Line Item). If freight charges apply, indicate if charges will be "per shipment" or "single one-time charge". Contractor shall supply a current published price list or GSA/VA Federal Supply Schedule contract, please submit contract number, expiration date, SIN, & MOL for supplies &/or services being procured along with your Commercial Published Price List. Refrigerated, frozen, or hazardous material must be indicated as such & a copy of the MSDS (Material Safety Data Sheet) must be supplied for any hazardous material. Specify any special handling instructions. Solicitation closes on Wednesday, September 23rd, 2015.
