Description
Statement of Work Supplemental Pest Control Service (Including Bed Bug Eradication) 1.Title of Project: Supplemental Pest Control Service for the Tampa VAMC, James A. Haley facility. 2.Scope of work: The purpose of this solicitation is for the Contractor to provide only On-Call Supplemental Pest Management Services (including Bed Bug eradication) for the Tampa VAMC, James A. Haley facility. The Contractor shall furnish all labor, supervision, tools, materials, equipment, and transportation necessary to provide Supplemental pest management services using Integrated Pest Management techniques (IPM) in accordance with contract requirements. *** The area not included in this request for On-Call Supplemental Pest Control Services are the Food and Nutrition, and Canteen areas. Pest Services The Supplemental pest services shall encompass as-needed eradication of indoor populations of rats, mice, wasps, cockroaches, ants, flies, spiders, and any other arthropod or common pest, including bed bugs. Estimated visits per year On-Call Supplemental Pest Services = 140 visits for the year Bed Bug On-Call Supplemental Pest Services = 100 for the year Services shall also include Pests that are located outside of the specified buildings, but within the property boundaries of the buildings, rats, mice, wasps, cockroaches, ants, flies, spiders, bed bugs and any other arthropod or common pest as directed by the COR. The Contractor shall also provide detailed, site-specific recommendations for structural and procedural modifications to aid in pest prevention that ensures the health and general well-being of patients, staff, and visitors as directed by the COR. All services shall be performed by a qualified Contractor who is licensed by the State of Florida, Florida Department of Agriculture and Consumer Services (FDACS), Bureau of Entomology and Pest Control, and legally authorized to perform pest control services. All services shall be completed in a timely manner and in accordance with state, local, federal, and VA standards as listed in the Performance Work Statement. Contractor will be contracted on an on-call basis for Treatment/Eradication, *** The area not included in this request for On-Call Supplemental Pest Control Services are the Food and Nutrition, and Canteen areas. Bed Bugs In the event of an outbreak or emergency, the contractor will provide immediate eradication response. Eradication of bed bugs shall include furniture, office spaces, and closets and so on. Contractor will conduct pest control services in accordance with Rule Chapter 5E-2 of the Florida Administrative Code & Florida Administrative Register (web address below). In addition, contractor will supply a list to the COR of all certified employees who will be on the premises along with a copy of their certification(s) no later than ten (10) days after contract award. These licenses and certifications shall be in compliance with Florida Statute Title XXXII, Chapter 482 (See below web addresses). http://www.freshfromflorida.com/content/download/3532/22113/Chapter%205E-2.pdf http://statutes.laws.com/florida/TitleXXXII/chapter482 Performance Requirements: A.The Contractor shall furnish all supervision, labor, materials, and equipment necessary to accomplish surveillance, trapping, treatment, pesticide application, and pest removal as listed above. B.The Contractor shall meet the performance expectations of the client in eradicating and suppressing the following pests: 1.Indoor populations of rats, mice, wasps, cockroaches, ants, flies, spiders, bed bugs and any other arthropod or common pest. 2.Pests that are located outside of the specified buildings, but within the property boundaries of the buildings including but not limit to, rats, mice, wasps, cockroaches, ants, flies, spiders, and any other arthropod or common pest C.The crawl spaces above the areas listed in this contract shall be treated on an as-needed basis. D.The contractor shall respond to all service calls from the COR within 4 hours after receipt of notice by telephone or email, twenty-four hours a day, seven days a week.: 1.Pest emergencies as identified by COR E.All areas listed in this contract must be equipped with "catch-all" mouse trapping stations designed to capture small pests prior to entry into facilities at any outside entrance. F.Corrective action shall be discussed and approved by the CO and the COR in writing and shall be taken as appropriate to control, prevent, or otherwise mitigate pest problems. G. The effectiveness of the service provided will be evaluated by the COR and corrective actions shall be taken until the specified levels of control are achieved. H.The Contractor shall provide and replace all expendable supplies (i.e. Rodenticide, Ant and Snail bait, Glue Boards) utilized in provide the service at no additional cost to the Government. I. The Contractor shall be required to notify the COR of any damaged or missing equipment for timely replacement by the Government. J.After each treatment, the Contractor shall submit a detailed report to the COR. The contractor shall provide basic knowledge/education/training about bed bugs to Government staff employees. 1.
