Description: Cleaning Service (Floor Mats) for entrances at Clarksburg VA Medical Center. Questions and Answers In order to ensure fair competition for this action. I will periodically provide updates concerning Questions received and Answers/Responses concerning this Combined Synopsis/Solicitation Notice. Q1: When we submit our quote the "amount " column cell in the "B.2 Price /Cost Schedule " should have the total for the 12 month ( 16 mat per week X 52 weeks ) is that's correct ? A1: Yes, the annual requirement entails 16 mats per 52 weeks IAW the terms and conditions listed in the solicitation. Q2: When and to whom should the RFQ be delivered? A2: Please send your responses to this RFQ to me at this email address NLT Friday, 1 pm (ET), 25 September 2015. Q3: Are there are 2 sets of mats, and is this is a pick up/ drop off situation, where the prior week's cleaned mats are traded out for the current week's dirty mats? A4: No, the organizations that is awarded the contract is expected to provide and exchange clean mats with dirty mats each week (IAW the terms and conditions listed in the solicitation). The clean mats (16 each) are the responsibility of the contractor. The mats requiring service (16 each) are located at the LAJVAMC. The customer expects that the contractor will conduct the cleaning at their own location and not at LAJVAMC. Q4: Are there any historical awards that are available for review? A4: No, this is a first time that this service has been placed on a contract. Precedence concerning this action is being established through this solicitation. Q5: Can this service be subcontracted? A5: Yes, awarded firms can subcontract to accomplish the work. Johnson VA Medical Center (LAJVAMC), located at 1 Medical Center Drive, Clarksburg, WV 26301. LAJVAMC shall not exercise any supervision or control over the contractor personnel performing the services herein. Contractor personnel shall be accountable solely to the contractor who, in turn, is responsible to the LAJVAMC. The contractor shall provide all personnel, equipment, supplies, transportation, materials, supervision, and non-personal services necessary to perform the entrance mat cleaning services. WORK STATEMENT FOR RECURRING SERVICES (a) Entrance mat cleaning services shall be provided once per week beginning the week of October 1, 2015 and ending no later than September 30, 2016 during LAJVAMC's normal operating hours. (b) Services will not be required or performed on the following federally observed holidays: New Year's Day, Martin Luther King, Jr.'s Birthday, George Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. (c) Contractor personnel will pick up the dirty charcoal-colored beveled-edged floor mats and replace them with clean charcoal-colored beveled-edged floor mats of the same size on a weekly basis at these locations throughout the hospital campus (there are a total of 16 mats currently): LocationMat SizeQuantity (i) Education entrance (1st floor):3' x 10' mat4 (ii) Outpatient entrance (1st floor):3' x 10' mat2 (iii) Stairway G (1st floor):3' x 10' mat1 (iv) Stairway H (1st floor):3' x 10' mat2 (v) Canteen patio entrance (1st floor):3' x 10' mat1 (vi) Biomed entrance (Basement):3' x 10' mat3 (vii) Building 5:3' x 10' mat1 (viii) Building 6:4' x 6'mat2 REPORTING (a) Contractor personnel will report to the Environmental Management Service Supervisor in basement Room #B138 during the beginning weeks of service commencement to be escorted around the hospital campus and shown the locations for the mat placements. (b) Upon completion of weekly services, contractor personnel shall provide the Environmental Management Service Supervisor with a list showing each mat serviced during the visit. The Environmental Management Service Supervisor will monitor contractor performance to ensure services performed in accordance with the requirements stated herein.
