- Sources Sought: This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 621610, Size Standard $15 Million. The Network Contracting Office at Canandaigua (VISN 2) is seeking a potential qualified contractor to provide Home Health Aide services for Veterans residing in or near Monroe County in the state of New York. Responses to this sources sought notice must include the following information: (1) General information about your company including name, address, contact person, phone number, fax number, email address(s), DUNS number and company website if available. (2) Type of business such as 8(a), small business, Hub Zone small business, Small Disadvantaged business, Service Disabled Veteran Owned small business, Veteran Owned small business, large business. (3) Does your company have experience with the Department of Veterans Affairs Home Health Aide Program? (4) What supplies/services does your company primarily provide? (5) Will your company be a joint venture with another company? (6) How long has your company existed? Responses to this market survey should be in writing and shall be faxed (315-425-4883) to the attention of Lisa Tompkins, Contract Specialist or emailed to Lisa.tompkins@va.gov. No telephone responses will be accepted. Responses must be received no later than 3:00 pm October 1, 2015. No solicitation document is available at this time; this notice is to acquire information only. Vendors interested in providing an offer will have to respond to a solicitation announcement that shall be published separately.
