Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The Department of Veterans Affairs, Network Contracting Office-8, in support of the Miami Veterans Healthcare System (MVAHS), Miami, Florida anticipates the award of a Firm Fixed Price Contract. The solicitation number is VA248-16-Q-0055 and is issued as a Request for Quote (RFQ) unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-60. The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a Small Business Administration (SBA) business size standard of 500 employees. This requirement is unrestricted and only qualified Offerors may submit bids. The solicitation pricing on www.fbo.gov will start on the date this solicitation is posted and will end on Friday, November 6, 2015 at 3:00 p.m. (EST). Free On Board (FOB) Destination shall be shipping information. This is to be a 100% SMALL BUSINESS (SB) SET-ASIDE. Offerors other than SB Concerns will not be considered. In addition to the price offers, the Offerors shall submit a signed copy of the SF 1449 in this solicitation via email to Brendalee.Clarke@va.gov not later than the Friday, November 6, 2015 at 3:00 p.m. Eastern Standard Time (EST). (a) The Contract Specialist will not accept any questions concerning this solicitation after Thursday, November 5, 2015, at 1:00 p.m. (EST). (b) Acknowledgment of Solicitation and Solicitation Amendments: This Solicitation is posted via www.fbo.gov. Submit signed and dated SF 1449 offers and SF 30 amendments posted, before the exact date and time specified in this solicitation. (c) Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All warranty, guarantees, and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. Vendor shall provide OEM letter approved by Manufacturer for authorization. (d) This procurement is for New Equipment ONLY; no remanufactured or "gray market" items. All Equipment must be covered by the manufacturer's warranty. New Equipment ONLY; NO remanufactured or "gray market" items. Bid MUST be good for thirty (30) calendar days after close of Buy. Offeror must be registered in the System for Award Management (SAM) database before an award can be made to them. If the Offeror is not registered in the SAM, it may do so through the SAM website at http://www.sam.gov. Potential offerors are responsible for monitoring this site for release of the solicitation package and any other pertinent information. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected Offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34.
