- This is a SOURCES SOUGHT NOTICE for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described in the Statement of Work (SOW). Potential offerors are invited to provide information via e-mail to susan.delacruz@va.gov. All responses will be used to determine the appropriate acquisition strategy for a potential future acquisition according to SOW. Ergonomics Assessment for VAPSHCS 1.This is an ergonomics assessment for VA Puget Sound Health Care System - Seattle and American Lake. The consultant shall be a Physical Therapist with special training in ergonomics. 2.Consultant shall familiarize themselves with the VA purchasing requirements and its restriction with ability to recommend the types of furniture or equipment as use ergonomically. This also includes recommending salient characteristics rather than specific manufacturers or models. 3.The consultant shall recommend furniture and equipment that meets the functional need of VA Puget Sound. The recommendation has to be ergonomically sounded and intended to improve employees' performance. The furniture and equipment shall be adjustable not limiting its use or its function; unless it applies specifically to employee(s) need due to medical evaluation by the qualified medical staff. The recommended furniture and equipment shall be priced reasonably and made durably. They shall check the National Acquisition Center (NAC) Contracts and GSA contracts prior to recommending an open market furniture and equipment. 4.HIPPA rules apply when consultant have access to protected information such as employees medical history. 5.When reporting assessment made for the individual or group work area, the report shall identify ergonomic risks with corrective action and recommendation. The individual or group work area shall be given numerous recommendations when practicable so that they can choose what fits their needs. The Occupational Health at the VA Puget Sound shall receive the summary whenever the individual report includes protected information, including health status of employees. However, the Supervisor and Safety Department receives only reports that do not include protected information of the individual, but shall include recommendation for correction. 6.Consultant shall provide the following services: a.Individual ergonomic consultations for designated employees: Consultant shall be put into contact with an employee or supervisor for scheduling. Consultant shall request, from the employee, medical documentation of any pre-existing injury or medical conditions of the designated employee that is relevant to ergonomics in that employee's workplace. Consultant shall schedule an onsite visit within 2 weeks, or later only if requested by the employee or supervisor. Consultant shall meet with the employee at their normal workplace(s) to observe them working there, and shall consult the employee and Supervisor on their needs, concerns, preferences and requests. The contractor shall develop recommendations to address any ergonomic needs of the identified workplace, and shall report as specified. It is estimated that these services may be requested at a rate of less than 2 per month. b.General work area consultations for work areas: Consultant shall meet with IH, with the Supervisor of the work area, and with selected employees in the work area to review injury history, work flow, needs of the department, and to observe work in the area. Medical documentation review may or may not be required. The contractor shall develop recommendations to address any ergonomic needs of the workplace, and shall report as specified. It is estimated that these services may be requested at a rate of less than 2 per year. c.Furniture and equipment purchasing guidelines: Consultant shall meet with FMS/Interior Design staff, IH, and other identified staff to recommend furniture needs and ergonomic-related equipment needs such as desks, chairs, keyboards, headsets, and pointing devices. VA Puget Sound must be able to set up standard purchasing agreements and contracts for identified furniture and equipment when the furniture or equipment needs of the facility change, additional consultations and recommendations shall be requested. This service may be requested one or more times per year. d.Design recommendations: As space remodels are planned, consultant may be asked to comment on the ergonomics of planned space designs. This service may be requested one or more times per year. 7.Cost estimates shall specify hourly costs for consultation, and shall specify a maximum cost for a single standard office ergonomic assessment with no review of medical records required, and another maximum cost with review of medical records. This maximum cost shall include all travel and reporting costs. Lowest price selection shall be based on these maximum costs. 8. The Estimated Period of Performance: December 1, 2015 thru November 30, 2016 and plus Four (4) Option Years period. Potential contractors shall provide, at a minimum, the following information to susan.delacruz@va.gov: 1) Name, address, point of contact name, phone number, and e-mail address. 2) Brief capability statement with enough information to determine if the company can meet the requirement. The Capabilities Statement for this sources sought is not expected to be a Request for Quote nor does restrict the Government to an ultimate acquisition approach, but rather short statement regarding the company's ability to provide the service above. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business Community or to conduct as an Unrestricted Procurement. Other than small businesses may respond to this notice in the event the market does not indicate SB interest, however, preference will be given IAW VAAR 819.7004 Contracting Order of Priority. The Government must ensure there is adequate competition among the potential pool of available contractors. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government nor will the Government pays for information solicited. This synopsis is not a solicitation announcement for Request for Quote. No formal solicitation document exists at this time, and no contract will be awarded from this announcement. Respondents will not be notified of the results of the evaluation. All interested contractors should notify this office via email by 4:00 PM (PDT) on November 6, 2015.
