- G.V. (Sonny) Montgomery VA Medical Center Jackson, MS 39216 BRIEF SCOPE OF WORK (This is for information only) Background The contractor shall provide all required labor, material and equipment, including travel cost to perform preventive maintenance on two GE Lightspeed RT 16 Hercules Tube (oil cooled) CT Scanners and associated workstations. Project Scope Performance: All work is to be performed by competent personal, experienced, trained, and qualified to work on equipment. All work performed shall be in accordance with manufacturer's instructions, including but not limited to: adjustments, calibrating, cleaning, lubricating, testing, disassembly, checkout, replacement of parts, etc. required to keep the equipment in operating condition. Contractor shall guarantee that all equipment covered by this contract shall be in optimum working condition at the expiration date. Parts: Contractor shall provide all new and OEM parts (including tube coverage) for repair of systems covered in this agreement. Contractor shall provide next day delivery by 1030 AM for any parts required. Unplanned Maintenance: Contractor shall provide on-site response when required within 4 hours of call. Remote Support: Contractor shall provide 24x7 system for submitting service requests. Contractor shall provide technical and remote services and support during standard hours from 8am to 5pm Monday through Friday. Planned Maintenance: Contractor shall perform preventive maintenance on systems covered in this agreement per manufacturer's recommendations. Up-time Guarantee: Contractor shall provide 98% uptime guarantee on devices. Location of performance G.V. (Sonny) Montgomery VA Medical Center 1500 E.
