ACKNOWLEDGMENT OF AMENDMENTS: The offeror acknowledges receipt of amendments to the Solicitation numbered and dated as follows: AMENDMENT NODATE B.2 LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUN 2011) This solicitation includes FAR 52.219-14 Limitations on Subcontracting. Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an "Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement" to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). (End of Clause) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the solicitation number is VA268-16-Q-0031. The government anticipated awarding a firm-fixed price contract resulting from this combined synopsis/solicitation. This solicitation is issued as a 100% set aside for small business concerns. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov. 1.0 CONTRACT DESCRIPTION AND REQUIREMENTS The Cleveland VA Medical Center has a requirement to purchase two (2) anesthesia delivery system/ pressure ventilator systems. The system requested shall be capable of providing ventilation to a wider range of animals and will condense the equipment used. This equipment will be used at the new Veterinary Medical Unit. A Firm Fixed Price contract will be awarded for the purchase of Brand Name or Equal equipment in accordance with all terms, conditions, provisions, specifications and schedule of this solicitation herein. Quotes shall contain the Offeror's best terms for cost/price and technical capabilities of the required Engler ADS 2000 Pressure Ventilator or equal equipment. The government reserves the right to award without discussions. 1.1 PRICING The Offeror shall submit price quotes for the required brand name or equal equipment, to the specifications listed in Section 1.2 below. Please enclose the established price list and description of capabilities and product specifications for the required equipment. The Offeror shall submit a separate price sheet for equal items to include Manufacturer Name, Stock/Part #, Unit of Issue and Price. Items offered that are brand name or equal shall identify items as being on schedule and reference the contract number for each item. Note: The Offeror should include a description of any support services or warranty information in the technical quote. 1.2 REQUIRED PRODUCTS & SPECIFICATIONS The system that we are requesting is necessary to upgrade and accommodate VA-funded research projects on campus. The research done at our facility now requires the use of monitoring anesthesia and ventilation of animals during surgery. There is a need for 2 - anesthesia delivery systems, microprocessor controlled anesthesia delivery system/positive pressure ventilators, 2 - anesthesia delivery system stands with V clamps and 2 vaporizers. 1.3 PRODUCT CAPABILITIES & SALIENT CHARACTERISTICS OR EQUAL The Engler ADS Pressure Ventilator System outlined below is one of the most important systems that is needed for the animal facility to function up to accreditation standards. Salient Characteristics: This request consists of the following components and must meet or exceed all specifications detailed as follows: Engler ADS Pressure Ventilator System: "Must be a stand-alone anesthesia/ventilator delivery system. "Must easily attach to a precision vaporizer and regulated oxygen source. "Must be able to be used without a vaporizer strictly as a ventilator unit. "Must ventilate and deliver anesthesia to a wide range of animals. "Must have an internal battery backup with the capability of operating as a portable unit for a period of 12 hours. "Must be preprogrammed with the necessary breathing parameters which controls a combination of electronic valves and electronic sensors that open only on the Aspiratory mode. "Ventilator parameters are fully programmed. "Must have positive pressure ventilation, Breathing Cycle, Inspiratory Time, Breaths per Minute, Flow Rate, Assist Setting, and Patient/Machine Initiated Breath "Must have Mask mode to maintain patient on mask "Must have breathe function, for a manually assisted breath at any time; "Must have fill and hold mode for Thoracotomy "Must have mechanical "pop off" valve, incorporated for additional safety "Must have "On the fly" adjustments of ALL parameters The product capabilities and salient characteristics identified above by a "brand name or equal" description are intended to be descriptive, not restrictive, and to indicate the quality and characteristics of the product that will be satisfactory. Quotes offering "equal" products (including products of the brand name manufacturer other than the one described by the brand name) will be considered if such product is clearly identified in the quote and is determined by the VA to meet fully the salient characteristics on above products. If offering an equivalent product, the brand name of the equivalent product shall be clearly identified in the quote. Unless visibly identified as an equivalent product, offered items shall be considered as the brand name product referenced in this announcement. The evaluation of quotes and the determination as to the equality of the products shall be the responsibility of the Government and shall be based on information provided by the Offeror. The Government is not responsible for locating or securing any information not provided in the quotation by the Offeror. To ensure that sufficient information is available, the Offeror must furnish, as a part of the quote, all descriptive material necessary for the purchasing activity to determine whether the products meet the salient characteristics of this requirement. Vendors proposing to modify a product to make it conform to the requirements of this announcement shall include a clear description of the proposed modification and clearly mark any descriptive material to show the proposed modification.
