Description
Background and Objectives The purpose of this contract is to obtain a qualified contractor to provide a recovery-oriented, multi-faceted and community-based facility to serve Veterans participating in the Substance Use Disorder Services (SUDS) formerly known as Alcohol and Drug Abuse Treatment Program (ADATP) - Dual Diagnosis Treatment Program (DDTP) for the James A. Haley Veterans Hospital (JAHVH). The contracted facility shall function as a form of supportive housing for homeless Veterans with co-occurring substance use and mental health disorders in need of supportive housing while enrolled in the SUDS continuum of care. The Contractor will demonstrate its ability to provide Halfway House services, validated by the JAHVH's inspection teams and MH&BSS coordinators/program managers, upon admittance of any referrals undergoing MH&BSS treatment, with an understanding that: "Halfway House beds- SUDS Halfway House Beds are specifically designated as post-treatment beds for Patients of the JAHVH who are homeless by federal definition, have completed substance abuse treatment, and require ongoing housing support to transition back to the community. The SUDS Discharge Coordinator, or designee, is responsible for the allocation of these beds. Interested Vendors The applicable NAICS for this service is 624229. To be considered for this requirement, vendors must be registered in the System for Award Management (SAM - https://www.sam.gov). Any vendor interested in this requirement should include the following information: 1.Company Information (business name, DUNS number, business size/category, GSA schedule if applicable) 2.Capability Statement (brief background of capabilities, resources, experience, etc.) 3.Contact information (POC name, email, and phone) 4.Describe firm's ability to provide the full range of services as outlined in the statement of work to include therapeutic and rehabilitative services, structured group therapy, room and board, meals and on-site laundry facilities. 5.Provide number of years the firm has been in business of providing community based health care to homeless individuals. 6.Describe firm's knowledge and understanding of the special needs of homeless veterans, chronic homelessness, chronic mental illness, and substance use disorders. 7.Describe firm's knowledge and experience in providing residential treatment and services for persons with severe and persistent mental illness and co-occurring substance abuse disorders. 8.Describe firm's methodology/approach to providing services to chronically mentally ill persons in residential settings demonstrating positive outcomes in response to services rendered. 9.Provide evidence of organization's compliance with Federal, State and local regulations governing the provision of health care. 10.Describe firm's capability/mechanism to begin contract performance within fourteen (14) days after award. 11.Specify location of proposed facility to be utilized under this contract. Include mailing address, street address, city, town, and county as applicable. 12.Provide information on the size and physical facilities to include number of beds. 13.Provide evidence of facility's conformity to the current Life Safety Code. 14.Provide copy of current occupancy permit. 15.Describe the ease of access for patients (e.g. located on bus route, parking space proximity, handicap accommodations, facility location in relation to major highways) and location in relation to the VA Medical Center. 16.Provide a list of the names Key Personnel to be utilized in the performance of this contract, along with a copy of their resumes/curriculum vitae, and a description of the particular services to be provided by such personnel. 17.Provide a staffing plan demonstrating that the contractor employs sufficient professional staff and other personnel to carry out the policies and procedures of the program.
