- Price Quote Template Solicitation Provisions and Contract Clauses SOW Attachment A - Service Area Diagram Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is HSBP1016Q0011 and the solicitation is issued as a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-83. Quotations are solicited only from small business concerns. The associated North American Industrial Classification System (NAICS) code for this procurement is 561790 "Other Services to Buildings and Dwellings" with a Small Business Administration (SBA) small business size standard of $7.5M in average annual receipts. Interested contractors that meet the SBA size standard may submit a response to this RFQ for snow and ice removal services to be performed at the U.S. Customs and Border Protection (CBP) United States Border Patrol (USBP) Detroit Sector Headquarters on Selfridge Air National Guard Base in Michigan. All snow and ice removal services shall be performed at USBP Sector Detroit in accordance with the attached Statement of Work (SOW). Due to the nature of this requirement and the critical importance of response time for the services sought, interested vendors must be located within 45 miles of Selfridge Air National Guard Base. The government intends to issue a purchase order for services with a base year period of performance and four option periods of one year each. A list of contract line items follows: CLIN Items Quantity Unit of Measure 010 Base Year Snow Removal 1 AU 020 Option Year 1 Snow Removal 1 AU 030 Option Year 2 Snow Removal 1 AU 040 Option Year 3 Snow Removal 1 AU 050 Option Year 4 Snow Removal 1 AU The provision at FAR 52.212-1, "Instructions to Offerors - Commercial Items", applies to this acquisition. The provision at FAR 52.212-2, "Evaluation - Commercial Items", does not apply to this acquisition, and the Government will evaluate quotations and issue an order to a responsive contractor whose past performance does not pose a risk to the Government, and whose quotation is the Lowest Price Technically Acceptable (LPTA). A quotation is technically acceptable if its technical capabilities conform to the Government's Statement of Work. In addition to pricing, each contractor must provide a technical proposal and three (3) past performance references. The technical proposal must demonstrate an understanding of the Government's requirements and show how the contractor will meet the requirements contained in the SOW. Past performance references must address the following information for each reference: Business/Client Name: Point of Contact Name: Phone #: E-mail (if applicable): Description of Work: Dollar Amount: The following provisions/clauses also apply to this solicitation: FAR 52.212-3, "Offeror Representations and Certifications - Commercial Items" - the selected contractor must submit a completed copy of the listed representations and certifications; FAR 52.212-4, "Contract Terms and Conditions - Commercial Items"; FAR 52.212-5, "Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items". Specifics regarding which additional FAR clauses cited in FAR 52.212-5, along with additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices are contained in the attached "solicitation provisions and contract clauses" document. ( See the attachment for specific information). The full text of the referenced FAR clauses may be accessed electronically at https://www.acquisition.gov/far/. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. A site visit for interested vendors to view the premises will occur on December 2, 2015 from 9:00 AM to 11:00 AM and 1:00 PM to 3:00 PM. To participate in the site visit, vendors should contact the Contracting Officer's Representative (COR) listed below to provide the necessary information to obtain Base access. Quotations are due by 4:00 PM EST on Friday, December 4, 2015 and may be submitted electronically to michael.c.amaral@cbp.dhs.gov.
