Intent to Sole Source **THIS IS NOT A REQUEST FOR COMPETITIVE QUOTES** The Department of Veterans Affairs VISN 19 intends to award a Firm Fixed Price contract in accordance with FAR Subpart 6.302-1; Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. This requirement is for a consignment of Maquet Hemashield Vascular Grafts. The NAICS code is 339112 with a size standard of 500 employees. The government intends to make a consignment award to: Maquet Medical Systems, 45 Barbour Pond Drive Wayne, NJ 07470-2094 720-879-2532 Award is to be made on or before December 15, 2015. The VA is obligated only to the extent of, and when use of the item is made by the VA. Unused (unopened and undamaged) items shall be returned, to the Contractor at the end of the effective period of the agreement, without reimbursement or other expenses to the Customer. This consignment agreement shall be effective for an estimated period of three (3) years from the date of signatures of both parties. 3. REASON THAT SUGGESTED SOURCE IS THE ONLY SOURCE, WHICH CAN PROVIDE THE SUPPLIES OR SERVICES: Maquet is the only vendor that produces and sells the Hemashield Knitted Vascular Graft.
