Description
Statement of Work The contract covers maintenance of the Elekta MOSAIQ software that is currently installed in the radiation therapy department at the VA Pittsburgh Healthcare System. In the event the software stops working the company will be required to intervene until the software is up and running. 5. Specific Tasks. 1. For a period of twelve (12) months, beginning upon the Effective date (the "Service Contract Period"), Elekta will use reasonable commercial efforts to assist Customer with the maintenance of the System in order to meet the requirements and specifications set forth in the Elekta Sales Quote, System Specifications and applicable Service & Installation Manual defined features and functions (the "Purchase Documentation"). 5.1 Task 1 - Enterprise Management Controls. 5.1.1 Subtask 1 - Integration Management Control Planning. Elekta will provide unlimited telephone and on-line support for questions during standard business hours. The company will provide on-site emergency repairs within 16 hours. All labor and parts are included in the contract costs. 6. Performance Monitoring The government reserves the right to inspect or test any supplies or services that have been tendered for acceptance. The COR will monitor radiologist user satisfaction to ensure contractor is meeting requirements. 7. Security Requirements If working on-site, contractor will be required to comply with physical security guidelines by either checking in with the VA Police each time they come on-site to perform contracted services or by obtaining a VA Contractor ID badge from the VA Police. VA ID badge must be worn at all times while on VA premises. Contractor owned computer equipment including laptops are not permitted to be connected to the VA network. The following language from VA Handbook 6500.6 is required in this contract: o Appendix D - Before being granted access to VA information or information systems, all contractor employees and subcontractor employees requiring such access must sign the VA's Contractor Rules of Behavior. Contractor must complete "VA Privacy and Information Security Awareness and Rules of Behavior" training prior to the performance of the contract and annually thereafter. Training must be completed in VA's TMS system (https://www.tms.va.gov/). Contractors must use the TMS Managed Self Enrollment method to complete the training in TMS. The COR must ensure that all contractors are validated in the PIH domain. Proof of training completion must be verified and tracked by the COR. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). NA 9. Other Pertinent Information or Special Considerations. NA a. Identification of Possible Follow-on Work. Any additional work that may result from completion of this contract, which may be added at a later date as a modification to this contract, or that may be helpful to the contractor in determining the best overall approach for the contract. b. Identification of Potential Conflicts of Interest (COI). NA c. Identification of Non-Disclosure Requirements. d. Packaging, Packing and Shipping Instructions. NA- already installed e. Inspection and Acceptance Criteria. NA- already installed 10. Risk Control NA- already installed. No infectious disease issues have been identified. 11. Place of Performance. Work will be performed remotely with the IT or radiology staff involvement. Location will be at 1010 Delafield Rd, Pittsburgh, PA 15215. 12. Period of Performance. Service will begin 12/1/2015 to 11/31/2016 13. Delivery Schedule. No deliverables are associated with the Maintenance contract. Maintenance support will remain/continue uninterrupted. 14.
