Description

THIS SOURCES SOUGHT IS NOT A REQUEST FOR QUOTATION: It is a market research tool being used to determine potential and eligible business firms that are capable of providing the services described herein prior to determining the method of acquisition and issuance of a Request for Quotation. The PSC for this requirement is 6515; the NAICS for this requirement is 339112 with a size standard of 500 employees. All interested businesses are encouraged to respond. The results of this Sources Sought will be utilized to determine if any Small Business Set-aside opportunities exist. All Small Business Set-aside categories will be considered. In addition, this market research tool is being used to identify potential and eligible firms, of all sizes, prior to determining the method of acquisition and issuance of a solicitation. If a solicitation is released, it will be synopsized on the Federal Business Opportunities (FedBizOpps) website. It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. The Government is not obligated to and will not pay for any information received from potential sources as a result of this synopsis. INTRODUCTION Requirement Overview: The devices requested are Neuroform Stent EZ System. The Neuroform Stent EZ System enables the endovascular treatment of brain aneurysms. The device is unique in its configuration allowing the treatment of wide neck aneurysms, particularly for those arising from smaller vessels. The SFVAMC is the leading VA center nationally for endovascular care of brain aneurysms and as such sees 10-20 aneurysms annually. Given the variation of aneurysm size, shape, and location a spectrum of stents are needed at all times. We are requesting 9 stents (3 x (2.5 mm x 20 mm); 3x (3.0mm x 20 mm); 1x (3.5 mm x 20 mm); 1x (4.0 mm x 20 mm), and 1x (4.5 mm x 20mm). The Neuroform stent is the only device, however, that is of an open-cell design and sized below 4.5 mm in diameter and approved for vessels as small as 2.0 mm. The former attribute permits better apposition of the stent in curved vascular anatomy as well as easier trans-stent catheter access, while the latter permits treatment of aneurysms arising from vessels distal the circle of Willis (e.g. middle cerebral artery) This information comes from the manufacturer instructions for use as approved by the FDA. That information comes from multicenter registries of both products, data that has been peer-reviewed, published, and available on Pubmed.gov REQUIREMENTS Compatibility requirements are: use with down vessel size of 2.0 mm and with XT-27 micro catheters that are already utilized at the VA San Francisco Medical Center. The Contract Type is anticipated to be a Firm Fixed Price (FFP), FOB Point: Destination; Delivery to: VA San Francisco Medical Center, 4150 Clement Street, San Francisco, CA 94121-1545. SUBMITTAL INFORMATION It is requested that interested businesses submit to the contracting office a brief capabilities statement package demonstrating ability to perform the services listed above. Additionally, in your response include a company profile to include number of employees, annual revenue history, office locations, DUNs number, etc. This documentation must address, at a minimum, the following: (1) Provide the type of work your company has performed in the past in support of same/similar requirement(s)? Include information such as contract number, organization supported, indication of whether a prime or subcontractor, contract value, Government point of contact and current telephone number, and a brief description of how the contract referenced relates to the technical services described herein; a. Describe the specific technical skill and experience which your company possesses which will ensure capability to perform the physical, functional, or performance tasks as outlined in the requirements above.
