Description

MTC Terms & Conditions Vendor Set-Up Form, W-9, Self-Certification SCA Wage Determinations Exhibit A Bid-Sheet Request For Quote THIS IS A SUBCONTRACTING OPPORTUNITY with Management & Training Corporation (MTC), operator of the Tongue Point Job Corps Center. MTC is requesting bids for bulk fuel prices of indefinite quantity. It is requested that the bidder will submit an average price per gallon for the estimated quantity to be used over the course of this contract. Items will be purchased on an as-needed basis throughout the year ending on December 31, 2016. Please provide a price quote including any delivery charges for the items listed on the attached Exhibit A bid sheet. Delivery terms and an estimated delivery lead time must also be specified. Details of the solicitation are in the attached request for quote (RFQ# TPJCC On-Going Bulk Fuel Delivery 15-44.pdf). All bids must be completed on the attached Exhibit A Bid sheet to be considered valid. In addition to the bid, all valid bids must include a W-9 and a completed MTC vendor self-certification form (attached). Bidders must self-certify under the listed NAICS code (NAICS codes can be found at http://www.census.gov/eos/www/naics/): This solicitation is listed under NAICS code: 424720 Petroleum and Petroleum Products Merchant Wholesalers (except Bulk Stations and Terminals) All bids are due Tuesday December 29 by 12:00 PM (PST).
