- SOURCES SOUGHT NOTICE The Department of Veterans Affairs, Network Contracting Office 16 is conducting market research to determine the Services Disabled Veteran Owned Small Business (SDVOSB), and Veteran Owned Small Business (VOSB) capabilities, for computer software license of a customized hospital discharge patient discharge booklet, relative to the NAICS 541511, size standard $27.5 million. This is a sources sought notice seeking information and sources that are capable of providing license software, support, the installation, implementation, training, and customization for an "After Hospital Care Plan" software. Information is requested to determine the availability of potential sources that are SDVOSB or VOSB, having the skills, and capabilities necessary to provide the customized software subscription, installation, training, and services support during period of performance. This is a sources sought only and is not a solicitation announcement. Responses to this notice will be used by the Government to make appropriate acquisition decision. After review of the responses, a solicitation announcement may be published on the www.FBO.gov website. Responses to this sources sought are not considered adequate responses to the solicitation that may be posted at a later date. There is no formal solicitation document at this time. The Government will not pay for any information provided for this source sought notice. At this time, Contracting Officer is only interested in capabilities that are in the category of SDVOSB and VOSB. The place of performance for this software subscription is for the John L. McClellan Memorial Veterans Hospital, (Also known as Central Arkansas Veterans Healthcare System), 4300 W. 7th St, Little Rock, AR 72205, and the second campus location Eugen J. Towbin Healthcare Center, 2200 fort Roots Drive, North Little Rock, AR 72114. The description of the software licensing that are being requested are for software that is creation of an "After Hospital Care Plan" that is comparable to the brand name, "Project RED", manufactured and licensing by the vendor, Engineered Care Inc. DESCRIPTION OF REQUIREMENT: The need for the comparable software to the brand name, Project RED, is to improve the hospital discharge process to promote patient safety and reduce re-hospitalization rates. One tool used with the "project" is the After Hospital Care Plan (AHCP) which is a color booklet designed to clearly present the information needed by patients to prepare them for the days between discharge and the first visit with their ambulatory care physician. The AHCP will replace our current Discharge Part B document with an easy to read, patient friendly discharge document that will make a meaningful impact on patient understanding of their discharge, patient satisfaction and patient safety. The capabilities of the successful vendor will license the use of the After Hospital Care Plan software to the Little Rock VA and support the installation, implementation, and customization of the After Hospital Care Plan software. The license grant includes, at no additional charge, the right to make and use a reasonable number of copies of the Software for testing, back-up, archival and disaster recovery purposes, and of the Documentation for internal training, support and deployment. A capable vendor will provide the following solution: "Creation of After Hospital Care Plans with health literacy appropriate patient discharge instructions for patients; "Report functionality to enable continual quality improvement and performance tracking; "Administrative interface to perform user management, application configuration and auditing; "Must be able to integrate the AHCP software with the hospital based VistA software used for evidence-based, VistA-Integrated Discharge Instructions. "Capable to replace the VA facility existing CPRS discharge note with a patient-friendly discharge instruction booklet that: * Has been shown to reduce unnecessary readmissions * Is customized for the VA environment to help educate veterans about their care and additional VA programs * Improves the discharge process at a VA site while providing leadership opportunities for clinical staff. Interface with CPRS to improve the discharge process. Allows the care givers to have one source to print Care Plans, the patients schedule appointments, etc. End-users will access via CPRS Tools. "Work with designated clinical and IT representatives of the Little Rock VA to deploy the VistA integrated AHCP. This work will include installation, configuration and customization as required. Salient Characteristics/Deliverables: The proposed software will include the following features: "Discharge planning software for providers "Creation of health literacy appropriate patient discharge instructions configured for optimal access by Little Rock VA staff "Reporting functionality to enable continual quality improvement and performance tracking "IT Professional interface to perform user management, application configuration and auditing "User accounts will be created for all users "Customized to match Little Rock VA branding requirements including logo placement "Addition of Little Rock VA related surgical discharge instructions Vendor will create and print a Care Plan booklet for every veteran discharged from the Central Arkansas Veterans Healthcare System site. The Care Plan will include the following data to the extent that they are available within the system: "Branded Site Logo "Patient Aligned Care Team details "Patient Demographics "Medications "Appointment Information "Diet and Activity Instructions "Additional Instructions "My HealtheVet information AHCP Software Installation and Configuration: AHCP Software setup. Install the software on the designated server system and create initial user accounts for testing. Implementation and Customization of After Hospital Care Plan Vendor will customize the After Hospital Care Plan for the Little Rock VA concerning specific medicines and procedures at CAVHS. The AHCP booklet will be customized to address/improve care delivery. INFORMATION REQUESTED and SUBMITTING: Vendors interested in this RFI may provide a response of your company capabilities statement. Submittals must be brief description, no more than five (5) pages, that demonstrates how our company has a similar product, and capability. Provide organization name, point of contract information, DUNS number, address, business size and type as validated via SAM, and SDVOSB and VOSB must be registered in the vetbiz registry https://www.vip.vetbiz.gov/ The Contracting Officer is only interested in capabilities of SDVOSB and VOSB vendors at this time. State if your product are under FSS or GSA contract or open market, if on schedule, please provide the contract number.
